目的研究补肾祛痰中药配合B超引导下经阴道未成熟卵泡穿刺治疗多囊卵巢综合征（PCOS）不孕症患者的临床疗效,探讨其对患者内分泌及基础窦卵泡数及血清抗苗勒氏管激素（AMH）的影响。方法对20例PCOS患者（51个周期）用克罗米芬/尿促性素（CC/HMG）促排卵后,当优势卵泡直径达到12～14 mm时,肌注绒促性素HCG 5000～10 000 U,32～36 h在超声引导下经阴道卵泡穿刺治疗,同时口服中药,水煎服,日一剂,连用2～3个周期,检查穿刺后患者的内分泌、卵巢基础窦卵泡计数和血清AMH水平的变化。结果治疗后PCOS患者睾酮（TO）和促黄体生成素（LH）、AMH及基础窦卵泡计数均明显降低,与治疗前比较差异有统计学意义（P〈0.05）。结论对PCOS患者用补肾化痰中药配合超声引导下经阴道未成熟卵泡穿刺技术可纠正其性激素及AMH异常状态,减少基础窦卵泡计数,从而为PCOS的治疗开创了一种简便易行、行之有效的新途径。

英文摘要：

Objective To research the clinical efficacy of the infertility with polycystic ovarian syndrome（PCOS）treated with Chinese medicine for tonifying the kidney and resolving phlegm in combination with transvaginal immature follicle puncture（TV-IMFP）under B ultrasonic sound and explore the impacts of this therapy on the endocrine,basic antral follicle count（bAFC）and serum anti-mullerian hormone（AMH）.Methods Clomiphene citrate（CC）/human menopausal gonadotropin（HMG）was administered for the ovulation induction for 20 cases of PCOS（51 cycles）.When the diameter of the dominant follicle was up to 12~14 mm,the muscular injection with human chorionic gonadotrophin（HCG）was done,5000~10 000 U.In 32~36 h,TV-IMFP under B ultrasonic sound was provided.At the same time,the herbal decoction was prescribed for oral administration,one bag a day,continuously for 2~3 cycles.The endocrinal condition,ovarian bAFC and serum AMH level were observed after puncture.Results After treatment,for the patients with PCOS,the levels of testosterone（TO）,luteinizing hormone（LH）,AMH and bAFC were all decreased obviously,presenting the statistical significant differences as compared with those before treatment（P〈0.05）.Conclusion The therapeutic program of Chinese medicine for tonifying the kidney and resolving phlegm in combination with TV-IMFP under B ultrasonic sound rectifies the abnormal states of sexual hormones and AMH and reduces bAFC for the patients with PCOS.It opens up a simple,convenient and feasible new approach to PCOS treatment.
