目的 探讨复发缓解型多发性硬化(RRMS)患者静息态脑运动网络的功能连接.方法 选取25例RRMS患者(RRMS组)和25名健康志愿者(对照组),采用3TMR扫描仪进行fMRI扫描.以双侧初级运动皮层(M1)为种子点,在Matlab7.8平台上采用静息态fMRI数据处理助手(DPARSF)、统计参数图(SPM)8、REST软件包对图像进行种子点功能连接处理,使用单样本t检验获取RRMS组与对照组各自的运动网络图,使用双样本t检验统计RRMS组和对照组脑运动网络的差异脑区.结果 基于种子点功能连接图,RRMS组和对照组静息态运动网络主要分布在双侧中央前后回、双侧辅助运动区(P<0.001);与对照组比较,RRMS组功能连接减弱的区域主要分布在扣带回、海马旁回及左小脑后叶(体素大小范围为43 ~ 273,t值为-4.9093 ~-3.2837,P值均<0.01),功能连接增强的区域主要分布在左中央后回、左辅助运动区、右枕中回、右舌回、左颞中回及右中央前回(体素大小范围为41~ 65,t值为2.4646~4.1263,P值均<0.01).结论 RRMS患者存在与静息态运动网络中心节点M1功能连接的异常改变.
Objective To analyze the changes of resting-state motor network in relapsing remitting multiple sclerosis(RRMS) patients.Methods Twenty-five patients with RRMS and 25 healthy control subjects were recruited in our study.Siemens 3 T MR scanner was used to obtain resting-state fMRI data.The data were processed with DPARSF,SPM8 and REST in Matlab 7.8.The one sample t test was performed for obtaining resting-state motor network diagrams of RRMS group and control group respectively.The two sample t test was performed to analyze the differences between RRMS group and control group.Results The brain areas of resting-state motor network were mainly distributed in bilateral precental gyrus,postcental gyrus and bilateral supplementary motor area(P<0.001),based on a seed functional connectivity method.Compared with control group,the reduced functional connectivity areas were cingulate gyrus,bilateral parahippocampal gyri,and left cerebellar posterior lobe in RRMS group(voxel size ranging from 43 to 273,t values from-4.9093 to-3.2837,P<0.01),the increased functional connectivity areas included left postcental gyrus,left supplementary motor area,right middle occipital gyrus,right lingual gyrus,left middle temporal gyrus and right precental gyrus(voxel size ranging from 41 to 65,t values from 2.4646 to 4.1263,P<0.01).Conclusion RRMS group has abnormal changes in the functional connectivity with the center node M 1 of the motor network in resting state.
