目的了解常驻高原部队官兵健康相关生命质量情况,探讨其与慢性高原病之间的关系。方法分层随机抽取某军区驻高原部队956个样本及该军区驻平原587个对照样本,实施欧洲五维健康量表(Europe health-related quality of life questionnaire 5 dimensions,EQ-5D)和慢性高原病(Chronic mountain sickness,CMS)检查。采用t检验、卡方检验、线性相关、非参数检验、方差分析等统计方法进行分析。结果高原组行动能力、疼痛或不舒服、焦虑或沮丧各维度评分均显著高于对照组(P〈0.05),EQ直观模拟标度尺(EQ visual analogue scale,EQ-VAS)评分显著低于对照组(P〈0.05)。行动能力、自我照顾、疼痛或不舒服、焦虑或沮丧各维度与CMS呈显著正相关(P〈0.05),EQ-VAS评分与CMS呈显著负相关(P〈0.05);疼痛或不舒服与心率和血红蛋白浓度呈显著正相关(P〈0.05),EQVAS评分与心率呈显著负相关(P〈0.05)。高原组中健康组、轻度CMS组、中重度CMS组各维度和EQ-VAS评分差异显著(P〈0.05)。结论常驻高原官兵的健康相关生命质量与CMS的发生有显著相关性。EQ-5D可以对高原官兵,尤其是CMS官兵的健康状况进行全面的评估,为高原卫勤保障措施的制定提供科学依据。
Objective To investigate the status of health-related quality of life in officers and soldiers stationed at high altitude, and explore its relationship with chronic mountain sickness (CMS). Methods A total of 956 subjects and 587 controls were sampled from the troops stationed at plateau and flatland respectively in a military command by stratified random sampling. Europe health-related quality of life questionnaire 5 dimensions (EQ-5D) and CMS diagnoses were performed on them. Student's t test, Chi- square, Correlation test, Nonparametric analysis, and ANOVA were used for analysis. Results The scores of mobility, pain/discomfort and anxiety/dispersion were significantly higher (P 〈 0.05 ), and the scores of EQ visual analogue scale (EQ-VAS) were significantly lower (P 〈 0.05 ) in the high altitude group than the control. The dimensions of mobility, self-care, pain/discomfort and anxiety/dispersion were positivelycorrelated with CMS ( P 〈 0.05 ). The scores of EQ-VAS were negatively correlated with CMS ( P 〈 0.05 ). Pain/discomfort had positive correlation with heart rate and hemoglobin concentration (p 〈 0. 05). The scores of EQ-VAS had negative correlation with heart rate (P 〈 0.05 ). There were significant differences in different dimensions and EQ-VAS score among those health, with mild and moderate-severe CMS from the high altitude group (P 〈 0.05 ). Conclusion Health-related quality of life is significant correlated with the occurrence of CMS in the officers and soldiers stationed in high altitude. EQ-5D can offer a comprehensive evaluation to the beahb state of the soldiers, especially those with CMS, and provide scientific evidences for the establishment of altitude medical service measures.
