目的:探讨针刺合谷穴引起的静息态脑功能连接的变化。方法:研究对象为健康志愿者20例,采用相同的MRI扫描序列分别采集针刺前、针刺中和针刺后的静息态功能数据。选择双侧后扣带回(PCC)、双侧第二感觉区(SⅡ)、双侧面部初级感觉区(SⅠF)、双侧手部初级感觉区(SⅠH)、双侧面部初级运动区(MⅠF)以及双侧手部初级运动区(MⅠH),共6个感兴趣区作为种子点。利用AFNI程序分析各感兴趣区静息态功能数据,并对各功能连接区针刺前、针刺中及针刺后的功能数据进行组内分析和组间(针刺中与针刺前、针刺后与针刺前)分析,对分析结果利用Monte Carlo方法进行多重比较校正。结果:组内分析结果显示,各感兴趣区均与脑内多个脑区存在着广泛性的功能连接。针刺中与针刺前组间分析显示各感兴趣区脑功能连接无明显变化;针刺后与针刺前组间分析显示,双侧SⅠH和双侧MⅠH针刺前后功能连接无明显变化,而双侧PCC、双侧SⅠF、双侧SⅡ以及双侧MⅠF则存在明显的功能连接增强或减弱。结论:针刺合谷穴能引起脑默认网络、面部感觉和运动网络的变化,说明合谷穴与面口部存在着密切联系,引发这些作用的最主要原因是针刺后效应。
Objective: To explore the acupuncture effect of Hegu point(LI4)on the brain functional connectivity.Methods: Study was carried out on 20 healthy right-handed volunteers.The same sequence of magnetic resonance scans was carried out and 3 resting-state fMRI data before,during,after acupuncture was acquired.Six regions of interest were selected as seed point to analyze the functional connectivity,including posterior cingulate cortex(PCC),secondary somatosensory cortex(SII),primary somatosensory cortex of face(SIF),primary somatosensory cortex of hand(SIH),primary motor cortex of face(MIF),and primary motor cortex of hand(MIH).All fMRI data was analyzed with AFNI program.Intra-group functional connectivity within groups before,during,after acupuncture were calculated,and inter-group functional connectivity between groups(during acupuncture and before acupuncture,after acupuncture and before acupuncture)were also calculated,and all results were corrected with Monte Carlo method.Results: Intra-group analysis showed that each region of interest had extensive functional connectivity with all the other region of brain.Compared with the functional connectivity before acupuncture,brain functional connectivity during acupuncture showed little significant change.However,functional connectivity after acupuncture for the most of region of interest,including PCC,SIF,SII,MIF,showed that there was significantly strengthened or weakened functional connectivity,except for the SIH and MIH.Conclusion: It was demonstrated that the brain default mode network,the sensorimotor network corresponding to face could be affected by Hegu acupuncture.It was inferred that Hegu point had close connection with face and the delayed effect might play important role in acupuncture.
