目的研究2型糖尿病患者血清25(OH)D_3(VD)水平及其与糖尿病视网膜病变(DR)的关系。方法对210例2型糖尿病患者根据眼底检查和眼底荧光血管造影结果将其分为无DR(NDR)组、非增殖型DR(NPOR)组及增殖型DR(PDR)组,并取同期35例正常人群作为对照。对每例患者进行临床资料收集及常规生化指标检测,采用电化学发光法检测所有对象血清VD水平。结果2型糖尿病患者中76.6%处于VD缺乏状态。随着2型糖尿病DR加重,血清VD水平呈下降趋势,差异有统计学意义(P〈0.05)。多因素Logistic回归分析发现较长的糖尿病病程(OR:2.379,P=0008)、高HbA1c(OR:5423,P=0.015)、高LDL-C(OR:2.922,P=O.039)、低血清VD水平(OR:0.776,P=O.003)是糖尿病DR的独立危险因素。结论2型糖尿病患者较正常人群存在更为严重VD缺乏。低VD水平、较长的糖尿病病程、高HbA1c与2型糖尿病患者DR的发生、发展密切相关。
Objective To estimate the status of 25-hydroxyvitaminD_3125(OH)D_3, VD] among type 2 diabetic adults and to assess the relationship between VD and diabetic retinopathy(DR), a key marker of diabetic microvascular dieases. Methods 210 type 2 diabetic patients and 35 non-diabetic healthy subjects in the same period were enrolled in this study. All the diabetic patients were divided into 3 groups: (1)NDR : non-DR, (2) NPDR: Non-proliferative DR, (3)PDR: Proliferative DR. Serum VD and were measured in diabetic patients and nondiabetic healthy subjects. Results Percentages of vitamin D debticiency [25 (OH) D_3〈50nmol/l] was 76.6%. Compared with the control group, serum VD were found to be decreased in diabetic patients (P 〈0.05). By multivariate logistic regression, the extent of diabetic retinopathy were significantly associated with long diabetic duration(OR: 2.379,P =0.008), HbA1c(OR:5.423,P =0.015), LDL-C(OR:2.922,P =0.039), serum VD(OR:0.776,P =0.003). Conclusion Vitamin D deficiency is much more common in the diabetic patients than non-diabetic healthy subjects. Servere diabetic retinopathy is significantly associated with an increased risk of decreasing level of serum VD, long diabetic duration and increasing level of HbAlc and LDL-C.
