目的探讨血清25羟维生素D3[25(OH)D3]水平与2型糖尿病患者下肢血管病变(LLPAD)的关系。方法选取210例2型糖尿病患者(T2DM组),根据下肢血管超声结果将患者分为LLPAD组(94例)和无下肢血管病变(Non-LLPAD)组(116例),并选取同期来本院体检的50例健康者为对照组。对每位患者进行临床资料收集及常规生化指标检测,采用电化学发光法检测所有受试者血清25(OH)D3水平。结果 LLPAD组患者血清25(OH)D3水平显著降低,与Non-LLPAD组比较差异有统计学意义(P〈0.05)。多因素Logistic回归分析发现年龄(OR=1.064,P=0.001)、吸烟(OR=3.639,P=0.001)、舒张压(OR=1.046,P=0.010)、糖化血红蛋白(HbA1c)(OR=1.188,P=0.048)、低密度脂蛋白胆固醇(LDL-C)(OR=1.520,P=0.028)、25(OH)D3(OR=0.096,P=0.001)是糖尿病下肢病变的独立危险因素。结论低维生素D水平、高龄、吸烟、高HbA1c、高舒张压、高LDL-C与2型糖尿病患者LLPAD的发生密切相关。
Objective To explore the relationship between 25-hydroxyvitamin D3 [25(OH)D3,VD] and lower limb peripheral arterial disease(LLPAD). Methods A total of 210 type 2 diabetic patients and 50 non-diabetic healthy subjects were enrolled in this study.All the diabetic patients underwent u1trasonography and were divided into 2 groups[LLPAD group(n=94) and non-LLPAD group(n=116)].Clinical data were collected,routine biochemical indices were determined,and serum VD were measured in the diabetic patients and the non-diabetic healthy subjects. Results VD levels decreased significantly in the LLPAD group,with statistically significant differences from the non-LLPAD group(P0.05).Multivariate logistic regression revealed that age(OR=1.064,P=0.001),smoking(OR=3.639,P=0.001),DBP(OR=1.046,P=0.010),HbA1c(OR=1.188,P=0.048),LDL-C(OR=1.520,P=0.028) and serum VD(OR=0.096,P=0.001) were independent risk factors for LLPAD among diabetic patients. Conclusion The development of LLPAD is closely related to old age,smoking,DBP,increased levels of HbA1c,LDL-C,and decreased level of serum VD.
