目的探讨表格式护理交接单在无缝对接护理中的应用。方法选取湖北中医药高等专科学校附属古城医院心内科2009年1月-2012年12月从冠心病临护病房转到普通病房的患者3894例，2009全年和2010全年分别转出患者880例和920例，这些患者在转接过程中，护理人员采用传统的方式书写交接护理记录；2011全年和2012全年分别转出患者1030例和1064例，对这些患者在转接过程中，护理人员采取填写表格式护理交接单的方式。对比采用表格式护理交接单前后交接记录书写时间、患者的满意度、护理质量评分以及护理不良事件发生的情况。结果经过临床观察，并将所得数据进行比较，表明与使用表格式交接单前相比，交接记录书写时间显著减少[（2．1±0．3）min／份]，患者满意度（97．7％）和护理质量明显提高，且护理不良事件（O％）显著减少。使用表格式护理交接单前后比较，差异有统计学意义（P〈0．05）。结论采用表格式护理交接单实现交接过程的无缝对接，达到了科学、高效、书写简便的功效，增强了患者转运的安全性，保证患者治疗的连续性，显著地提高患者的满意度和护理质量．受到了广大护理人员的亲睐，值得在临床上进行推广应用。

英文摘要：

Objective To investigate the clinical application of tabular nursing delivery sheet in seamless docking nursing. Methods A total of 3894 patients transferred from CCU to the general ward in the cardiology department of Affiliated Gucheng Hospital of Hubei Junior College of Traditional Chinese Medicine from 2009 to 2012 were selected. For the 880 patients and 920 patients were transferred in January 2009 and December 2010 respectively, the nursing staffs used the traditional method to write the hand-over nursing records. For the 1030 patients and 1064 patients transferred in 2011 and 2012, the nursing staffs used the tabular nursing hand-over sheet. The writing time of hand- over records, patient satisfaction, scores of nursing quality and occurrence of nursing adverse events before and after the application of tabular nursing hand-over sheets were compared. Results After clinical observation, the obtained data were compared, which indicated that compared to before the application of tabular nursing hand-over sheets, the writing time of hand-over records shortened significantly [（2.1±0.3） min/copy], the patient satisfaction （97.7%） and the nursing quality improved significantly, and the nursing adverse events （0%） reduced significantly. There existed significant difference between before and after the application of tabular nursing hand-over sheets, with statistical sig- nificance （P 〈 0.05）. Conclusion The application of tabular nursing hand-over sheets realizes the seamless hand-over during the transfer process, reaches the scientific, efficient and convenient effect, increases the safety of patient trans- fer, guarantees the continuity of patient nursing, remarkably improves the patient satisfaction and quality of nursing and is favored by the nursing staffs, thereby by worth clinical popularization and application.
