目的探讨高强度聚焦超声(HIFU)损伤不切肋骨山羊肝脏组织的可行性。方法31只羊随机分成两组,实验组经静脉注入乳化碘油,用HIFU对肝脏进行辐照,比较坏死组织体积、能效因子、靶区灰度及组织病理检查。对照组以normal saline代替乳化碘油,余步骤同实验组。结果两组损伤组织均为凝固性坏死。实验组损伤率、靶区灰度阳性率分别为71.1%和26.8%,对照组分别为16.3%和4.1%(P〈0.05)。结论乳化碘油可增强HIFU的损伤效果,通过一定的手段实现在不切肋骨条件下对肝脏组织的HIFU损伤是可行的。
Objective The purpose of this study was to explore the feasibility of ablating goat liver using HIFU and a self-made iodinated lipid emulsion (SMILE) through intercostal spaces. Methods Thirty-one goats were randomly divided into two groups. SMILE was injected intravenously into goats in the experimental (Ex) group. Goats' livers were exposed to HIFU through the intercostal spaces half an hour after SMILE was injected intravenously. The enhanced gray scale in target region was real-time recorded. The sizes of the lesions were measured and energy efficiency factors were calculated after experiment. Tissues at the lesion boundary were sampled for histological examination. All procedures were the same as those for the Ex group except that SMILE was replaced with normal saline in the control (con) group. Results Typical necrosis occurred in the target issues. The necrosis rates were 71. 1% and 16.3% in Ex and con groups respectively(P〈0.01). Conclusions Necrosis rate of goats liver may be increased by HIFU combined with SMILE. It may be reliable to ablate liver tissue in large animals using HIFU combined with some approaches through the intercostal spaces.
