目的利用功能性核磁共振成像(f MRI)技术评估三叉神经痛患者处于静息状态下大脑自发性功能活动变化。方法选择原发性三叉神经痛(ITN)患者组26例,健康对照组22例,受试者静息态下进行功能性核磁共振扫描。利用DPARSF软件对图像进行处理,得到两组人员的全脑平均低频振幅率(mf ALFF)值。使用SPM12和SPSS 17.0软件分析数据。结果两组受试人员在年龄、受教育程度和男女比例上差异无统计学意义;ITN患者组部分脑区mf ALFF值明显高于健康对照组,分别是:左侧枕中回(BA18)、枕下回(BA19)、三角部额下回(BA47)、梭状回(BA37)、小脑脚1区、距状裂周围皮层、中央旁回,右侧背外侧额上回(BA8),差异有统计学意义(P〈0.01)。结论 ITN患者多个相关脑区的mf ALFF值增高,提示患者在静息状态下,部分脑区的活动改变可能与疼痛的调节或感觉的整合有关。
Objective To evaluate the brain function changes of patients with idiopathic trigeminal neuralgia (ITN) by functional magnetic resonance imaging (fMRI) under the resting-state. Methods 26 patients with ITN were involved in ITN group and 22 healthy adults were selected as control group. Both groups were examined by fMRI in the resting state. Images were processed with data processing assistant for resting-state fMRI(DPASF) and mean fractionalamplitude of low frequency fluctuations (mfALFF) were acquired in the two groups. SPM12 and SPSS 17.0 were used for statistic analysis. Results There were no significant differences in age, educational level and gender ratio between the two groups. The mfALFF in ITN group was higher than that in control group in the left middle occipital gyrus, inferior occipital gyrus, pars triangularis gyrus frontalis inferior, fusiform gyrus, cerebellum crust 1, calcarine fissure and surrounding cortex, paracentral lobule, and the right dorsolateral superior frontal gyrus(P 〈0. 01 ). Conclusion In patients with primary trigeminal neuralgia, mfALFF increases in multiple regions of the brain at rest, and the mechanism may be associated with pain regulation or sensory integration.
