目的 探讨多排CT多期增强扫描胰腺癌图像特征的多样性.方法 回顾性分析120例经穿刺或手术证实的胰腺癌患者的多排CT图像和临床资料,分析其影像学特点.结果 根据平扫病灶与周围胰腺组织的密度差异分为3组：第1组为等密度,66例（55％）;第2组为界清低密度,1 7例（14.2％）;第3组为界不清稍低密度,37例（30.8％）.3组患者间病灶部位、病灶动脉期、平衡期强化程度及正常胰腺组织门脉期强化程度无明显统计学差异,而3组间病灶的直径、病灶门脉期、胰腺组织动脉期、平衡期强化程度存在统计学差异.3组内病灶与胰腺组织三期强化程度差异均无明显统计学意义.3组间肿瘤相关胰腺炎、血管侵犯无明显统计学差异,3组间胆管扩张、主胰管扩张、远端胰腺组织萎缩程度及区域淋巴结、远处转移间存在统计学差异.结论 胰腺癌的多排CT多期增强扫描呈现出多种多样的表现,密度、发生部位及间接征象多样,但是乏血供是其最主要的特点.

英文摘要：

Objective To discuss the variability of multiphasic contrast-enhanced multi-detector CT findings of pancreatic adenocarcinoma.Methods By retrospective analysis 120 cases confirmed pancreatic adenocarcinoma by biopsy or operation,the performance of multiphasic contrast-enhanced multi-detector CT was reviewed.Results 120 cases were divided into three groups by the difference of density between pancreatic adenocarcinoma and the pancreatic parenchyma.The first group was isoattenuating：66 cases （55％）,the second group was hypointense with clearly defined borders：17 cases（14.2％）,the third group was mild hypointense with unclearly defined borders：37 cases（30.8 ％）.Among the three groups,no significant difference in the location of the lesion was observed.No significant differences in the degree of enhancement of lesions in the arterial phase,balance phase and normal pancreas in portal venous phase were observed,whereas significant differences in the degree of enhancement of lesions in portal venous phase and normal pancreas in the arterial phases,balance phases were observed among three types,significant difference in the diameter of the lesion was observed among three types.In each group,no significant differences in the degree of enhancement of lesions and normal pancreas in the three phases were observed.No significant differences in tumor-associated pancreatitis and vascular invasion were observed among three groups,the significant differences in the atrophy degree of diatal pancreas,lymph node and distant metastasis and dilatation of the main pancreatic duct and the bile duct were observed among the three groups.Conclusion Pancreatic cancer shows a varied performance on multi-detector CT multi-phase comtrast enhanced images.Although it＇ s density,location and indirect signs varied,the lack of blood supply is its main feature.
