目的探讨低氧诱导因子-1(HIF-1α)、Smad蛋白的相互作用蛋白1(SIP1)和基质金属蛋白酶-9(MMP-9)在甲状腺乳头状癌(PTC)中的表达及意义。方法采用免疫组化SP法检测72例人甲状腺乳头状癌和37例癌旁正常组织中HIF-1α、SIP1和MMP-9的表达水平,分析其表达与临床病理指标的关系及三者间表达的相关性。结果在PTC组织中,HIF-1α、SIP1和MMP-9的阳性表达率分别为51.56%、51.39%和54.16%,显著高于癌旁正常组织(P〈0.01)。HIF-1α、SIP1和MMP-9的表达与患者颈部淋巴结转移显著相关(P〈0.05)。HIF-1α与SIP1、HIF-1α与MMP-9、SIP1和MMP-9的表达均呈显著正相关(rs=0.304,P=0.009;rs=0.243,P=0.040;rs=0.277,P=0.019)。2种蛋白(HIF-1α/SIP1,HIF-1α/MMP-9,SIP1/MMP-9)联合表达较二者之一表达与颈部淋巴结转移更具相关性(P〈0.05)。HIF-1α、SIP1和MMP-9 3种蛋白联合表达较三者之一表达与颈部淋巴结转移更具有相关性(P〈0.05)。结论 HIF-1α、SIP1和MMP-9在PTC组织中的表达与颈部淋巴结转移密切相关,且三者存在相互协同作用,其表达情况可能作为监测PTC颈部淋巴结转移的指标。
To investigate the expression and significance of HIF-1α,SIP1 and MMP-9 in papillary thyroid carcinoma(PTC),we analyzed the expressions of HIF-1α,SIP1 and MMP-9 in 72 PTC samples by immunohistochemistry.Furthermore,the correlations of HIF-1α,SIP1 and MMP-9 expression with one another,and with several clinicopathological indicators were statistically analyzed.In PTC samples,the positive expression rates of HIF-1α,SIP1 and MMP-9 were 51.56%,51.39% and 54.16% respectively,which were significantly higher than those in the normal thyroid group(P 0.01);HIF-1α,SIP1 and MMP-9 expression had significant correlations with cervical lymph node metastasis(P〈 0.05).Meanwhile,HIF-1α expression had a positive correlation with SIP1(rs=0.304,P=0.009) and MMP-9(rs=0.243,P=0.040),wihle SIP1 and MMP-9 expression were positively correlated(rs=0.277,P=0.019).Concomitant expression of any two or all of the three molecules had stronger correlation with lymph node metastasis than did each alone(P〈 0.05).In conclusion,HIF-1α,SIP1 and MMP-9 in PTC samples work synergistically and closely correlated to cervical lymph node metastasis.Therefore,the expression of HIF-1α,SIP1 and MMP-9 might be used as biomarkers for cervical lymph node metastasis of PTC.
