目的蒽环类药物作为淋巴瘤患者的主要化疗用药,可造成不可逆的心脏损害。本研究旨在以平衡法核素心血池显像(ERNA)为标准,探讨实时三维超声心动图(RT-3DE)评价拟行蒽环类药物化疗的淋巴瘤患者左心室收缩功能(LVSF)的准确性及其临床应用价值。资料与方法回顾性分析2012年12月—2014年9月于复旦大学附属肿瘤医院确诊的51例淋巴瘤患者,于化疗前分别行RT-3DE及ERNA检查,测定左心室射血分数(LVEF),分析RT-3DE与ERNA测得LVEF的相关性和一致性。结果 RT-3DE较ERNA所测LVEF平均高估0.35%,但差异无统计学意义(P〉0.05);经相关分析,RT-3DE与ERNA所测LVEF有较好的相关性(r=0.559,P〈0.001);经Bland-Altman一致性检验,RT-3DE与ERNA所测LVEF具有一致性。结论 RT-3DE能够较为准确地测量LVEF,且操作便捷、价格相对低廉、无放射性损伤,可广泛应用于肿瘤化疗患者的心功能随访。
Purpose Anthracyclines,as primary chemotherapy drugs in patients with lymphoma,can cause irreversible cardiac toxicity.The purpose of this study is to explore the accuracy and clinical value of real-time three-dimensional echocardiography(RT-3DE) in left ventricular systolic function(LVSF) assessment in lymphoma patients underwent anthracycline chemotherapy,using equilibrium radionuclide angiography(ERNA) as the reference standard.Materials and Methods Fifty-one lymphoma patients were enrolled into this retrospective study during the period between December 2012 and September 2014.All patients were confirmed without cardiovascular disease and received RT-3DE as well as ERNA before chemotherapy.The left ventricular ejection fraction(LVEF) calculated with two different examination methods were underwent the correlation analysis as well as the consistency check.Results For the measurement of LVEF,RT-3DE was overvalued by an average of 0.35% compared with ERNA.There was no significantly statistical difference between RT-3DE and ERNA(P0.05).Good correlation were validated between RT-3DE and ERNA(r=0.559,P0.001).Furthermore,LVEF derived from RT-3DE and ERNA achieved consistency by the Bland-Altman consistency check.Conclusion RT-3DE is an accurate measurement for LVEF.Given these advantages such as convenient operation,relatively cheap and no radiation injury,RT-3DE can be widely used in evaluating the left ventricular systolic function on patients accepting chemotherapy in the regular follow-up.
