目的:从疾病和证候两个层面对冠心病复合高血脂状态的大鼠模型进行证候属性的多时间点动态评价。方法:模拟临床冠心病发病特点,采用高脂饮食联合左冠脉结扎术制备模型。术后7、14、28d,动态检测血脂、心电图、超声心动等疾病指标,旷场实验、应激状态下呼吸频率、足底RGB值、体质量、体质量增量、BMI、24h进食量等宏观体征进行证候判定。结果:冠心病复合高血脂状态模型在术后7d心电图检测呈现6-8个病理性Q波纳入研究,术后7、14、28d,TC、TG、LDL-C均显著升高(P〈0.05,P〈0.01)。超声心动显示EF值、FS值均显著降低,LVID;d、LVID;s显著升高(P〈0.01),旷场3min自主运动距离显著下降(P〈0.01),应激状态下呼吸频率显著升高(P〈0.05,P〈0.01);足底RGB均值和R值显著降低(P〈0.05,P〈0.01);体质量和进食量先降后升,至术后28d体质量显著升高(P〈0.05,P〈0.01),自术后14d始出现体质量增量和BMI的显著升高(P〈0.05,P〈0.01)。结论:冠心病复合高血脂状态大鼠模型在术后7-14d属于气虚血瘀证,14-28d过渡为气虚血瘀兼有痰浊证。
Objective: To dynamically evaluate syndrome property from disease and syndrome in coronary heart disease(CHD) rats complicated with hyperlipemia. Methods: According to clinical characteristics of patients with CHD, the model was established by high fat diet(HFD) and the ligation of left anterior descending(LAD) artery. Dynamic detection of serum lipids, electrocardiogram(ECG), echocardiography, open-field test, respiratory rate under stress, RGB value of planta pedis, body weight, body weight growth, BMI and daily-intake were collected to assess syndrome property on the 7th, 14 th and 28 th day after operation. Results: ECG was performed on the 7th day in the model of CHD complicated with hyperlipemia, and 6-8 pathologic Q waves were collected in this research. TC, TG, LDL-C levels increased at the 7th, 14 th and 28 th day after operation. The echocardiogram showed that ejection fraction(EF), fraction shortening(FS) significantly decreased, while the left ventricular internal diameter at end-diastole(LVID;d) and left ventricular internal diameter at end-systole(LVID;s) significantly increased. Meanwhile, the 3 min independent movement distance decreased in the open-field test and the respiratory rate under stress increased obviously. The RGB value and R value of Planta pedis significantly decreased. The body weight and daily-intake went up after dropping, and body weight reached a significant high till the 28 th day, and from the 14 th day, body weight growth and BMI began to increase remarkably. Conclusion: CHD complicated with hyperlipemia rat model could be established by HFD combined with LAD. This model belongs to syndrome of qi-deficiency and blood stasis during the 7-14 th days after operation with a transition to syndrome combined with turbid phlegm during the 14-28 th days. It could provide a stable model foundation for further biological basic investigation of TCM syndromes and efficacy evaluation of traditional Chinese Medicine.
