目的探讨2型糖尿病周围神经病变(DPN)患者外周血白细胞介素6(IL-6)的改变及其临床意义。方法选取100例2型糖尿病患者,分为糖尿病周围神经病变组(DPN组)50例和无糖尿病周围神经病变组(NDPN组)50例,另选择50例健康人群作为对照组(NC组)。测定外周血IL_6含量,比较三组间IL-6水平差异;应用Pearson相关分析,探讨影响血清IL-6浓度的相关影响因素;应用Logstic回归分析,了解DPN发病的独立影响因素。结果(1)T2DM纽IL-6水平显著高于NC组,DPN组IL-6水平显著高于T2DM组(均P〈0.01);(2)Pearson相关分析显示,血清IL-6水平与运动及感觉神经传导速度均呈负相关(r值分别为-0.488,-0.389,均P〈0.05),与胰岛功能指数呈负相关(r=-0.384,P〈0.01)。Logistic回归分析发现体质量指数、收缩压、IL-6为DPN发病的独立危险因素。结论2型糖尿病并发周围神经病变患者外周血IL-6水平明显增加,与DPN发病密切相关。
Objective To explore the changes of serum interleukin 6 (IL-6) levels in patients with type 2 dia- betes peripheral neuropathy (DPN) and its clinical significance. Methods A total of 100 patients with type 2 diabetes were divided into diabetic peripheral neuropathy (DPN)group (50 cases)and non-diabetic peripheral neuropathy (NDPN) group(50 cases). In addition,50 cases of healthy people were selected as control group (NC group). The ser- um level of IL-6 were detected and compared among three groups. Pearson correlation analysis was used to explore which factors influence on the serum level of IL-6. Logistic regression analysis were carried out to evaluate the independent in- fluencing factors of DPN. Results ( 1 ) Serum level of IL-6 was higher in NDPN group than that in NC group , Serum level of IL-6 was higher in DPN group than in NDPN group (P 〈0.01 ). (2) Pearson correlation analysis showed that the serum level of IL-6 was negatively correlated with both motor and sensory nerve conduction velocity ( r = - 0. 488, r = -0.389 ,respectively,P 〈 0.05 ), and also negatively correlated with islet function index (r = -0.384 ,P 〈 0.01 ). Logistic analysis showed that body mass index, systolic blood pressure and IL-6 were independent risk factors for DPN. Conclusion The levels of IL-6 are increased significantly in patients with type 2 diabetes peripheral neuropathy and closely related to the development of DPN.
