目的检测类风湿关节炎(RA)患者外周血CCR6+CD4+T细胞芳香烃受体(AhR)水平,探讨AhR在RA发病机制中的意义。方法采用流式细胞术测定33例RA患者及14例健康对照者外周血CCR6+CD4+T细胞中AhR阳性细胞比例,分析其与RA患者临床指标的关系。结果 1)RA患者组外周血单个核细胞(PBMCs)中CCR6+CD4+T细胞占(10.75±2.92)%,高于健康对照组(4.96±2.28)%,差异有统计学意义(t=6.581,P〈0.01)。2)RA患者组内外周血CCR6+CD4+T细胞中AhR阳性细胞所占比例(46.02±14.68)%,高于CCR6-CD4+T细胞(21.62±6.94)%和PBMCs(34.22±10.33)%中AhR阳性细胞占比,差异均有统计学意义(z=-6.432,P〈0.01;t=-3.776,P〈0.01)。3)RA患者组外周血CCR6+CD4+T细胞中AhR阳性细胞所占比例(46.02±14.68)%,高于健康对照组(23.18±5.62)%,差异有统计学意义(z=-4.909,P〈0.01)。4)相关性分析:RA患者组外周血CCR6+CD4+T细胞中AhR阳性细胞所占比例与血沉(ESR)、C反应蛋白(CRP)、抗环瓜氨酸肽(抗-CCP)抗体、DAS28评分、疾病病程均无相关性(P均〉0.05)。结论 AhR阳性细胞在RA患者外周血CCR6+CD4+T细胞中比例高,可能参与了RA发病,但其水平高低不能反映RA疾病活动程度。
This study aimed to explore the possible mechanism of artemisinin involved in the treatment of lupus nephritis and clarify the regulatory role of dihydroartemisinin (DHA) on fractalkine (FKN) expression in lupus nephritis by studying the effect of dihydroartemisinin on the FKN and NF-κB expression in the renal cortex of lupusprone MRL/lpr mice. Thirty-two female MRL/lpr mice at 12 weeks of age were randomly divided into four groups, which received a daily intragastrie administration for 8 weeks of DHA (DHA-treated mice), prednisone (prednisone- treated mice), DHA and prednisone (DHA+prednisone mice) and normal saline (control mice), respectively. Then renal pathological changes were observed by optical microscopy; the expression of FKN and NF-κB p65 mRNA in renal cortex were detected by RT-PCR. Expression of FKN and NF-κB p65 protein in renal cortex were detected by immnnohistochemical staining. Data showed that the FKN, NF-κB p65 mRNA and protein were expressed in the renal cortex of MRL/lpr mice. And the reduced proteinuria, improved kidney function and ameliorated renal lesions were found in the DHA-treated mice. Furthermore, the expression of FKN, NF-κB p65 mRNA and protein in the renal cortex of MRL/lpr mice were down-regulated by DHA treatment. In conclusion, DHA can alleviate lupus nephritis by regulating the expression of FKN and NF-κB, especially in combination with prednisone.
