[目的]分析延边农村地区朝鲜族和汉族居民胰岛素抵抗(IR)与代谢综合征(MS)及其组分的相关性.[方法]采用整群抽样和简单随机抽取的方法抽取7836名延边朝鲜族自治州农村地区常住朝鲜族和汉族进行现况调查.采用家访的方法收集研究对象的一般情况、心脑血管疾病的既往史及家族史等相关信息.通过健康体检准确测量身高、体质量、腰围、臀围、血压等体格指标.取清晨空腹(禁食12h)静脉血,测量胰岛素、血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL—C)、血糖等生物化学指标.[结果]不同性别汉族IR组的整体型肥胖、腹型肥胖、高TG血症、低HDL—C血症、高血压、高血糖及MS患病风险高于非IR组,其差异均有统计学意义(P〈0.01);不同性别朝鲜族IR组的上述诸多代谢异常(低HDL.C血症除外)的患病风险高于非IR组,其差异均有统计学意义(P〈0.01).偏相关分析结果显示,IR与整体型肥胖、腹型肥胖、高TG血症、低HDL.C血症、高血压、高血糖及MS均明显呈正相关,相关系数分别为0.161,0.137,0.125,0.120,0.074,0.422,0.196(P〈0.01).[结论]延边农村地区不同民族IR组的整体型肥胖、腹型肥胖、高TG血症、低HDL—C血症、高血压、高血糖及MS患病风险高于非IR组.
OBJECTIVE To analyze the relationship between insulin resistance (IR) and metabolic syndrome (MS) and its components in Korean-Chinese and Han nationality residents in countryside of Yanbian. METHODS 7 836 cases of permanent residents in countryside of Yanbian were selected by chester sampling and simple random sampling to perform the inventory survey. The relevant data for this study were collected by family visit and health physical examination, including the general information, past history and family history of cardiovascular and cerebrovascular diseases, etc, and anthropometric indices (height, weight, waist and hip circumference) and blood pressure. In addition, the biochemistry indexes of insulin, serum total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C) and blood glucose were measured from early morning venous blood after 12h fasting. RESULTS The risks of suffering from the overall obesity, central obesity, hypertriglyceridemia, low HDL-C, hypertension,hyperglycemia and MS in Korean-Chinese (except low HDL-C) and Han nationality were significantly higher in IR group than in non-IR group (P〈0.01), and the partial correlate analysis showed that there was a significant positive correlation between IR and overall obesity, central obesity, hypertriglyceridemia, low HDL-C, hypertension, hyperglycemia and MS, the partial correlate coefficient was 0. 161, 0. 137, 0. 125, 0. 120, 0. 074, 0. 422 and 0. 196 (P〈0.01). CONCLUSION The risks of suffering from the overall obesity, central obesity, hypertriglyceridemia, low HDL-C, hypertension, hyperglycemia and MS in different nationalities of countryside of Yanbian are significantly higher in IR group than in non-IR group.
