目的从冠心病患者冠状静脉窦中取血检验含凝血酶敏感素4型基序的解聚素金属蛋白酶(ADAMTS4)及高敏C反应蛋白(hs-CRP),评估经皮冠状动脉介入治疗(PCI)对ADAMTS4及hs-CRP水平的影响。方法根据冠状动脉造影结果,将81例患者分为对照组、简单病变组及复杂病变组,其中30例患者接受了PCI治疗。血液检测标本从冠状静脉窦获得。ADAMTS4及hs-CRP值分别通过ELISA及免疫浊度法检测。结果复杂病变组ADAMTS4及hs-CRP值明显高于对照组及简单病变组(P〈0.001)。所有研究对象及冠心病患者的ADAMTS4水平均与hs-CRP水平相关(r_1=0.73,r_2=0.76,P〈0.01)。PCI后ADAMTS4值高于PCI前(P〈0.001),hs-CRP亦表现出相似结果 (P=0.025)。结论冠心病患者冠状动脉ADAMTS4及hs-CRP水平升高。PCI后冠状动脉ADAMTS4及hs-CRP升高,可能由PCI过程中球囊扩张或支架植入的机械挤压致冠状动脉斑块破裂释放而出。
Aim To detect the level of a disintegrin and metalloproteinase with thrombospondin motifs 4( ADAMTS4) and high sensitivity C-reactive protein( hs-CRP) using coronary sinus sampling and to assess the effect of PCI on ADAMTS4 and hs-CRP levels. Methods The 81 subjects were classified according to their coronary angiographic findings into control,simple,and complex groups. PCI was performed in 30 patients. Blood samples were taken from the coronary sinus. ADAMTS4 and hs-CRP levels were measured by ELISA and turbidometry respectively. Results ADAMTS4 showed a higher level in complex group( 100. 1 ± 41. 3 μg/L) than in control( 55. 9 ± 17. 9 μg/L) and simple group( 79. 5 ± 34. 8 μg/L). The ADAMTS4 level showed a positive correlation with the hs-CRP level whether from all patients or from patients with CAD( r1= 0. 73,r2= 0. 76). The ADAMTS4 value was higher after PCI than before PCI( 108. 9 ± 39. 5 μg/L vs 84. 4 ± 35. 3 μg/L,P〈0. 001),and the same was found regarding hs-CRP( 3. 1 ± 1. 4 mg/L vs2. 6 ± 1. 2 mg/L,P〈0. 001). Conclusion Coronary ADAMTS4 level elevated in patients with CAD. The increase of ADAMTS4 and hs-CRP levels after PCI might be attributed to their release from the coronary atheroma.
