目的:比较全冠修复后的下颌第一磨牙与天然同名牙颊、舌面所受口内压力的差异。方法:纳入面部对称、单侧下颌第一磨牙需全冠修复的患者15名,并对其受试牙进行全冠修复,同时使用个性化压力测量装置分别测量全冠修复体及其对侧天然同名牙的颊、舌面所受的压力;借助硅橡胶轻体材料记录修复牙位的中性区位置后,用齿科扫描仪对全冠戴入前后的下颌牙列模型进行扫描,并用逆向工程软件测量两者间的差异。1周后随访,由患者通过VAS表评价全冠颊、舌面的异物感。结果:天然同名牙颊舌面平均受压分别为(2.50±1.36)g/cm2、(3.33±1.83)g/cm2,平均舌/颊受压比1.38±0.29;全冠修复体的颊舌面平均受压分别为(2.22±2.03)g/cm2、(4.40±3.77)g/cm2,平均舌/颊受压比2.17±0.52,其平均舌/颊受压比显著高于天然牙(P〈0.05)。全冠修复体的颊、舌面的受压情况与其偏离中性区位置的距离间均呈显著正相关(P〈0.05)。46.67%的受试者评价全冠修复体颊舌面有异物感。结论:下颌第一磨牙经全冠修复后,其颊舌面的形态改变了基牙口内颊舌向的受力。
AIM: To compare the buccolingual pressures between natural mandibular first molar and its homonym crown. METHODS: 15 participants with facial symmetry who needed crown restoration of the mandibular first molar were enrolled in this study. Buccal and lingual pressure of the natural teeth and the homonym crowns was measured using pressure sensor on the try - in visit. The neutral zone position of the mandibular first molars was also recorded for further comparison with the corresponding position of the restoration. VAS form was graded by the partici- pants for the foreign body sensation on axial contour of the restoration 1 week after crown cementation. RESULTS : The buccal and lingural pressure of the natural teeth was (2.50 + 1.36) g/cm2 and( 3.33 + 1.83 ) g/cm2, that of the homo- nym crown was (2.22 ± 2.03 )g/cm2 and(4.40± 3.77 )g/cm2, respectively. The lingual/buccal pressure ratio of the crown was significantly higher than that of the natural tooth (2.17 ±0.52 vs. 1.38 ±0.29, P 〈0.05). Positive corre- lation was observed between buccolingual pressures and the buccolingual displacement of the neutral zone of the restora- tion. 46.67% of the participants graded foreign body sensation at both sides. CONCLUSION: The modality induced buccolingual force balance of the crown restoration is the main factor of the comfort level.
