CO2点阵激光联合微波治疗嵌甲、甲沟炎28例

目的 观察CO2点阵激光联合微波治疗嵌甲、甲沟炎的疗效。方法 56例中、重度嵌甲、甲沟炎患者随机分为2组,治疗组28例,其中12例中度病变患者运用CO2点阵激光切割病变侧甲并将其拔除,然后用微波烧灼、破坏病变侧甲母质。16例重度病变患者除激光拔甲、微波甲母质治疗外,对甲沟软组织化脓、增生性肉芽组织予以微波或超脉冲CO2激光烧除进行甲沟重建; 对照组28例,全部仅采用CO2点阵激光切割病变侧甲并将其拔除。 结果 中度病变患者治疗组及对照组治愈率均为100.00%。重度病变患者治疗组治愈率87.50%,对照组治愈率50.00%,治疗组与对照组比较差异有统计学意义(P〈0.05)。结论 CO2点阵激光切割拔甲治疗中度嵌甲、甲沟炎及其联合微波治疗重度嵌甲、甲沟炎简单、有效,创伤小,复发率低。

英文摘要:

Objective To seek for a simple, effective and less traumatic therapy for ingrown nail and paronychia.Methods Fifty-six patients with moderate to severe ingrown nail and paronychia were randomly divided into the treatment group(n=28)and the control group(n=28). In the treatment group, affected nails in 12 cases with moderate lesion were removed with lattice CO2 laser, followed by destroying the nail matrix with microwave. In addition to given treatments with laser and microwave, the other 16 cases with severe lesion in the treatment group were also treated with microwave or ultrapulse CO2 laser to remove purulent and hyperproliferative granulation tissues in the nail groove to reconstruct the nail groove. In the control group, affected nails in 28 cases were removed with lattice CO2 laser.Results All nails in moderate cases were cured in both the treatment and the control groups. In the severe cases, cure rates in the treatment and controls were 87.50% and 50.00%, respectively(P〈0.05). Conclusion The treatment of moderate ingrown nail and paronychia with lattice CO2 laser to remove nail, and of severe ingrown nail and paronychia with lattice CO2 laser combined with microwave are simple and effective therapies with less traumatic and low recurrence rate.
