目的探讨吸烟对男性精液质量的影响并分析不育患者精子活力和动态参数的相关性。方法应用伟力彩色精子质量分析系统对255例男性不育患者(吸烟组96例、非吸烟组84例和烟酒组75例)和112名正常生育男性的精子活力、直线运动速度(VSL)、曲线运动速度(VCL)、平均路径速度(VAP)、平均移动角度(MAD)、侧摆幅度(ALH)、鞭打频率(BCF)、前向性(STR)、直线性(LIN)和摆动性(WOB)等参数进行检测并分析其相关性。结果与正常对照组相比,吸烟组、非吸烟组和烟酒组的精子活力、VSL、VCL、VAP、ALH、LIN和STR显著降低(P〈0.05、P〈0.01),而BCF显著升高(P〈0.05、P〈0.01)。吸烟组的精子活力显著低于非吸烟组(P〈0.05、P〈0.01);烟酒组精子活力、VSL、VCL、VAP、MAD和ALH显著低于吸烟组(P〈0.05、P〈0.01),而BCF显著升高(P〈0.01)。大烟量组精子活力、VSL、VCL和ALH显著低于小烟量组(P〈0.05、P〈0.01),而BCF显著升高(P〈0.01)。长烟龄组精子活力、VSL、VCL、VAP、ALH和LIN显著低于短烟龄组(P〈0.05、P〈0.01)。吸烟男性不育患者精子活力与VSL、VCL、VAP、LIN和STR呈明显正相关(P均〈0.05),与BCF呈明显负相关(P〈0.001)。结论吸烟可引起男性精子活力和精子动态参数异常,可能是导致男性不育的独立因素。烟量和烟龄对男性精子活力和动态参数的不良影响存在量效和时效关系,大量吸烟和烟龄长者可能对男性不育有不良影响。平均VSL、平均VCL、VAP、LIN、STR和BCF是反映吸烟男性不育患者精子活力的有效指标。
Objective To study the influence of cigarette smoking on the sperm quality and the correlation between the motility of sperm and the kinesis parameters in infertile males.Methods A total of 255 infertile males(96 smokers,84 non-smokers and 75 smokers with drinking)and 112 fertile controls were enrolled in the study and were determined by WEILI colour semen analysis system.The sperm kinesis parameters,including sperm motility,velocity of straight line(VSL),velocity of curvilinear(VCL),velocity of average path(VAP),migratory degree(MAD),amplitude of oscillation(ALH),beat cross frequency(BCF),straightness(STR),linearity(LIN) and wobble(WOB),were determined,and their correlations were also analyzed.Results Compared with the controls,there was a significant decrease in the sperm motility,VSL,VCL,VAP,ALH,LIN and STR(P0.05,P0.01),and there was a marked increase in BCF in the infertile smokers,non-smokers and smokers with drinking(P0.05,P0.01).The sperm motility was much lower in the infertile smokers than in the infertile non-smokers(P0.05,P0.01).The sperm motility,VSL,VCL,VAP,MAD and ALH were significantly lower in the infertile smokers with drinking than in the infertile smokers(P0.05,P0.01),while BCF increased obviously(P0.01).The sperm motility,VSL,VCL and ALH in the small-quantity and short-time smoking group were also higher than those in the big-quantity and long-time smoking group(P0.05,P0.01),and BCF increased in the small-quantity smoking group(P0.01).Moreover,VAP and LIN in the long-time smoking group decreased significantly than in the short-time smoking group(P0.05,P0.01).There were significant positive correlations between the sperm motility and VSL,VCL,VAP,LIN and STR(P0.05).Furthermore,there was a significant negative correlation between the sperm motility and BCF(P0.001).Conclusions Cigarette smoking is remarkably associated with abnormal sperm motility and sperm kinesis parameters.Smoking may be an independent risk factor f
