上海市女性骨量变化与骨质疏松检出率

目的在了解上海市女性骨密度(BMD)变化规律基础上,探讨使用不同年龄段峰值BMD作为诊断标准时,比较骨质疏松检出率的差异。方法采用Hologic QDR-2000双能X线吸收仪测量3390例20-84岁女性腰椎1-4(L1-4)后前位和左侧股骨近端(Total、Neck、Troch和Inter)BMD值,确定峰值BMD出现的年龄段,在此基础上统计分析骨质疏松检出率。结果以5岁年龄段分组分析显示:L1-4峰值BMD出现在30—34岁年龄段,为0.989g/cm^2,股骨近端各部位峰值BMD均出现在20.24岁年龄段,分别为0.878g/cm^2、0.806g/cm^2、0.655g/cm^2和1.019g/cm^2;以上述峰值BMD作为标准时,2317例50-84岁妇女L1-4、Total、Neck、Troch和Inter骨质疏松检出率分别为42.2%、15.1%、13.4%、9.4%和13.1%。而以20-39岁年龄段BMD作为峰值分析显示:L1-4、Total、Neck、Troch和Inter均值BMD分别为0.962g/cm^2、0.844g/cm^2、0,776g/cm^2、0.626g/cm^2和0.984g/cm^2;2317例50-84岁妇女L1-4、Total、Neck、Troch和Inter骨质疏松检出率分别为29.3%、18.3%、14.9%、16.0%和16.1%。以5岁年龄段分组与以20-39岁年龄段BMD作为峰值比较,骨质疏松检出率在L1-4、Total、Troch、Inter部位差异均有统计学意义,而Neck部位差异无统计学意义。结论以不同年龄段BMD作为峰值时,骨质疏松检出率存在差异,临床以BMD来诊断骨质疏松时,应关注该问题。

Objective To compare the prevalence of osteoporesis by using different age-interval BMD as the diagnostic criteria on the basis of age-related changes in BMD from Shanghai women. Methods The BMD in the postereanterior lumbar spine 1-4 and left proximal femur(including Total, Neck, Troch and Inter) was measured using Hologic QDR-2000 dual energy X-ray absorptiometry (DXA) in a total of 3 390 women aged 20 - 84 years old. To select different age-interval BMD as peak BMD and calculate the prevalence of osteoporosis. Results Crossing-sectional data analysis in stratified 5-year age intervals revealed that the peak BMD of pesteroanterior lumbar spine 1-4 occurring at 30 - 34 years was 0.989 g/cm^2 and left proximal femur (Total, Neck, Trech and Inter) appearing at 20 - 24 years were 0.878 g/cm^2 ,0.806 g/cm^2 ,0.655 g/cm^2 and 1.019 g/cm^2, respectively. The prevalence of osteoperesis in 2 317 subjects ranging from 50 - 84 years were 42.2 % at Spine, 15.1% at Total hip, 13.4% at Neck,9.4% at Trech,and 13.1% at Inter, respectively. Using the mean BMD of the 20 - 39 years old group as the peak BMD revealed that the value of BMD in L1-4 and left proximal femur(Total, Neck ,Trech and Inter) were 0.962 g/cm^2, 0.844 g/cm^2 ,0.776 g/cm^2 ,0.626 g/cm^2 and 0.984 g/cm^2, respectively. The prevalence of osteoporesis in 2 317 subjects ranging from 50 - 84 years were 29.3% at Spine, 18.3% at Total hip, 14.9% at Neck, 16.0% at Trech,and 16.1% at Inter, respectively. Comparison of the two methods showed that statistical differences existed in the prevalence of osteoporesis at most skeletal sites. Conclusion Statistical differences existed in the prevalence of osteoporosis using different age-interval BMD as the diagnostic criteria. To diagnose osteoporosis in clinic, this issue should be, taken into consideration.
