目的:探讨抑癌基因FHIT表达及人乳头瘤病毒16(HPV16)的基因型整合状态在宫颈癌发生发展中的作用及相关性。方法:选取柳州市人民医院2013年6月至2014年12月收治的42例宫颈癌、55例宫颈上皮内瘤样病变(CIN)和20例宫颈正常组织的患者,免疫组化法检测宫颈组织中FHIT蛋白表达;多重PCR法检测HPV16 E2/E7表达。结果:FHIT蛋白的总阳性表达率为57.26%(67/117),正常宫颈组织、CINⅠ、CINⅡ、CINⅢ和宫颈癌中FHIT蛋白阳性率分别为85.00%、80.00%、75.00%、60.00%和26.19%。随着宫颈病变加重,FHIT蛋白阳性表达率下降,组间差异有统计学意义(χ^2=7.335;P=0.003)。117例单纯HPV16阳性标本HPV16总整合率为81.20%,正常宫颈组织、CINⅠ、CINⅡ、CINⅢ和宫颈癌中整合率分别为60.00%、66.67%、75.00%、95.00%和92.86%;随病变加重,整合率增强,组间差异有统计学意义(χ^2=5.713,P=0.003);FHIT蛋白阳性表达在不同HPV16整合时不同,差异有统计学意义(χ^2=11.989,P=0.000)。结论:HPV16基因整合可能通过诱导FHIT基因低表达从而促使宫颈癌发生发展。
Objective: To explore the expression of fragile histidine triad(FHIT) and its relationship with the integration of human papilloma viral 16(HPV16) in cervical cancer.Method: The physical status of HPV16 E2/E7 were detected by Multiplex PCR(Polymerase chain reaction) in 42 patients with cervical cancer,55 patients with CIN and 20 patients with normal cervix tissues.And immunohistochemistry S-P assay was used to detect the expression of FHIT before operation.Results: The total positive expression rate of FHIT was 57.26%.The positive expression rates of FHIT were respectively 85.00% 、80.00% 、75.00% 、60.00% and 26.19% in normal cervix tissues,CINⅠ,CINⅡ,CINⅢ and cervical cancer tissues.The difference was statistically significant(χ~2= 7.335,P = 0.003).The total integration rate of HPV16 was 81.20% in 117 only HPV16 positive cervical specimens.The integration rate of HPV16 were respectively 60.00% 、66.67% 、75.00% 、95.00% and 92.86% in normal cervix tissues,CINⅠ,CINⅡ,CINⅢ and cervical cancer tissues.And its showd an increasing trend in different cervical lesions(χ~2= 5.713,P = 0.003).Between the total integration of HPV16 and the total descending expression of FHIT were obvious difference(χ~2= 11.989,P = 0.000).Conclusion: There are close relationship of HPV16 genome integration correlates with the descending expression of FHIT in the progression of cervical lesion.
