目的观察坐骨神经分支损伤(sNI)大鼠接受脉冲射频(PRF)治疗后机械痛痛阈的变化。方法将雄性SD大鼠40只随机分为5组:健康对照组、SNI组、假SNI组、PRF组、假PRF组,每组8只。其中,SNI组接受坐骨神经分支损伤手术,建立SNI模型;假SNI组仅暴露坐骨神经分支而不切断;PRF组在建立SNI模型7d后,实施PRF治疗(脉冲频率为2Hz、交流电频率为500kHz、输出电压为45V、治疗时间120s);假PRF组建立SNI模型7d后,仅置入射频针120s,不实施PRF治疗。于D0(建立SNI模型前)、D1~D7(建模后1~7d),D8~D14(PRF后1~7d),用von Frey丝刺激大鼠术侧后爪诱发触痛过敏的方法评估各实验分组大鼠疼痛行为。结果D0时各组机械痛阈值比较差异无统计学意义(P〉0.05)。D7时,SNI组、PRF组、假PRF组较健康对照组疼痛阈值显著降低(P〈0.05);假SNI组与健康对照组比较差异无统计学意义(P〉0.05)。D9与D14时,PRF组较SNI组痛阈明显增高(P〈0.05);PRF组较假PRF组痛阈明显增高(P〈0.05);PRF组与健康对照组、假SNI组比较差异无统计学意义(P〉0.05)。结论PRF治疗后的SNI模型大鼠机械痛阈升高。
Objective To observe the change of paw withdrawal threshold in the rat model with spared sciatic nerve injury (SNI) after the treatment by pulsed radiofrequency(PRF). Methods Forty male SD rats were randomly divided into five groups(n=8) : control group, SNI group, SNI sham group, PRF group and PRF sham group with 8 rats in each group. The rats in group SNI were received SNI of sciatic nerve; the sciatic nerve branches were exposed only in SNI sham group; group PRF were treated at 42 ℃ with a pulse duration of 20 ms (500 kHz) and pulse rate of 2 Hz to the proximal dorsal root ganglion(DRG) for 120 s on the 7th postoperative day; PRF sham group were received needles implantation only. Paw withdrawal threshold was measured before SNI procedure (Do), 1-7 d after model(D0 -D7 ), and 1-7 d after PRF(D8 -D14 ). Results No significant differences of paw withdrawal threshold were found among all groups at Do (P〉0.05) ; Compared with control group, paw withdrawal threshold of SNI group, PRF sham group and PRF group were significantly lowered at D7 (P〈0.05) ; There was no significant difference between sham SNI group and control group(P〉0.05) ; The paw withdrawal threshold of PRF group was significantly increased compared with SNI group and PRF sham group at D9 and D14 (P〈0.05) ;PRF group was no significant differ- ence compared with control group and SNI sham group at D9 and D14(P〉0.05). Conclusion The pave withdrawal threshold is increased significantly in SNI model rat after the treatment by PRF.
