目的 了解华中科技大学同济医学院附属荆州医院(以下简称“我院”)38个临床科室使用便携式血糖仪进行血糖监测的质控现状,为制订院内血糖监测质控管理提供依据。方法 对我院血糖监测实施规范化管理培训及考核结束后1个月,第1次采用自行设计的便携式血糖仪对全院38个使用便携式血糖仪进行血糖监测的临床科室进行质控。结果 97.37%的科室更换新批号试纸条时未行质控品检测及登记;92.11%的科室未做到每天血糖检测前进行质控品检测并登记;76.32%的科室试纸未注明开封日期、时间及责任人;65.79%的护士在进行多人采血时,未使用快速手消毒剂进行手的消毒;63.16%的护士未将一次性采血针弃于锐器盒内;68.42%的护士没有帮助患者查找血糖正常或异常的原因。结论 我院38个临床科室使用便携式血糖仪进行血糖监测时,质控较欠缺,38名护士临床使用便携式血糖仪监测血糖操作合格,但在手的消毒、医疗废物处理以及对糖尿病患者相关知识的宣教上仍不到位,因此要加大这几个方面质控的力度。
Objective To investigate the current situation of pol:table glucometer used in 38 clinical departments of Jingzhou Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology ("our hospi- tal" for short) to conduct monitor of blood glucose, and to provide basis for formulating the quality control management of nosocomial blood glucose monitoring. Methods One month after the hospital implements training and assessment of blood glucose standardized monitoring; a self-designed quality control table was used to record the result of portable blood glucose quality control. This method was applied when using portable blood glucose to monitor quality control in 38 clinical departments of the hospital. Results 97.37% of the departments did not conduct quality control test and registration when replacing a new test strip lot; 92.11% of the departments did not give a daily quality control test and registration before blood glucose testing; 76.32% of the departments did not label the opening date, time and responsi- ble person; 65.79% of the nurses did not use the fast hand disinfectant when performing multi-people blood collection; 63.16% of the nurses did not discard the disposable lancets in sharp objects boxes; 68.42% of the nurses did not help patients find the reason why the blood sugar was normal or abnormal. Conclusion 38 clinical departments in our hospi- tal have some deficiencies in blood glucose monitoring quality control. 38 clinical nurses are qualified when using portable blood glucose meters to monitor the operation, but in the aspects of sterilization, medical waste disposal, as well as the education of diabetes knowledge, these nurses are not qualified. Therefore, we should increase the intensity of quality control in those aspects.
