目的探讨女性生殖生育因素与系统性红斑狼疮(SLE)发病的关系,为SLE的病因学研究及一级预防提供理论依据。方法采用病例对照的方法,选取天津医科大学总医院感染免疫科的121例SLE患者及在妇科就诊的非SLE患者122例作为对照。调查SLE患者及对照者的一般资料、月经生育情况等。采用单因素和多因素非条件Lo-gistic回归分析对SLE危险因素进行分析。结果单因素非条件Logistic回归分析结果显示,流产手术〔OR=7.53,95%CI(3.09,18.36)〕、绝育手术〔OR=30.04,95%CI(3.88,232.92)〕、月经量〔OR=2.39,95%CI(1.12,5.16)〕、月经规律情况〔OR=4.05,95%CI(1.67,9.82)〕、出生时母亲的年龄〔OR=1.08,95%CI(1.02,1.15)〕、初潮年龄〔OR=0.67,95%CI(0.57,0.80)〕、绝经年龄〔OR=0.55,95%CI(0.42,0.71)〕与SLE的发病相关(P〈0.05)。多因素非条件Logistic回归分析显示,流产手术〔OR=4.28,95%CI(1.55,11.79)〕、初潮年龄〔OR=0.68,95%CI(0.50,0.91)〕、绝经年龄〔OR=0.51,95%CI(0.39,0.67)〕、月经规律情况〔OR=2.88,95%CI(1.13,7.31)〕与SLE的发病相关。结论月经初潮与绝经年龄早,做过流产手术,月经不规律,均是SLE的危险因素,这些影响可能潜在于SLE的病因机制中。
Objective To examine the associations between female reproductive and menopausal factors and the development of systemic lupus erythematosus(SLE),in order to provide the etiology and clues for primary prevention of SLE.Methods A case-control study was conducted on 121 case and 122 controls from the general hospital of Tianjin Medical University by using univariate and multivariate unconditional Logistic regression model to analyze the relative risk factors like physical data,menopausal factors.Results Univariate unconditional Logistic regression model showed that induced abortion(OR=7.53,95%CI(3.09,18.36)),sterilization operation(OR=30.04,95%CI(3.88,232.92)),menstrual flow(OR=2.39,95%CI(1.12,5.16)),menstrual irregularity(OR=4.05,95%CI(1.67,9.82)),mother′s age when born(OR=1.08,95%CI(1.02,1.15)),age at menarche(OR=0.67,95%CI(0.57,0.80)),age at menopause(OR=0.55,95%CI(0.42,0.71)) were associated with SLE(P0.05).Multivariate unconditional Logistic regression model showed that the risk factors of SLE including induced abortion(OR=7.53,95%CI(3.09,18.36)),age at menarche(OR=0.67,95%CI(0.57,0.80)),age at menopause(OR=0.51,95%CI(0.39,0.67)),menstrual irregularity(OR=4.05,95%CI(1.67,9.82)).Conclusion Early age at menarche and menopause,induced abortion and menstrual irregularity were associated with SLE.These associations may be involved in the mechanisms underlying the pathogenesis of SLE.
