目的探讨帕金森病(PD)认知功能障碍及病情程度与血糖水平的关系。方法给140例PD患者进行神经心理学量表检查和Hoehn。Yahr(H—Y)分级评定;以及空腹血糖(FPG)、餐后2h血糖(2hPPG)和糖化血红蛋白(HbA1c)水平检测。并与正常对照者比较。结果本组PD患者中认知功能正常70例(认知正常组),有认知功能障碍70例(认知障碍组);H—Y分级〈3级(H.Y分级〈3级组)90例,H—Y分级≥3级(H-Y分级I〉3级组)50例。与正常对照组比较,PD认知功能正常组、认知功能障碍组及H—Y分级〈3级组、t〉3级组的FPG和HbA1c水平明显升高(均P〈0.05);2hPPG水平的差异无统计学意义。而PD认知障碍组与认知正常组、H-Y分级〈3级组与≥3级组的FPG、2hPPG和HbA1c水平的差异均无统计学意义。结论有、无认知功能障碍及不同病情程度PD患者的血糖水平均升高。PD认知功能障碍和病情程度可能与血糖水平没有明显的关系。
Objective To explore the relationship between cognitive dysfunction, condition and level of plasma glucose in Parkinson disease (PD). Methods Neuropsychology scale and Hoehn-Yahr (H-Y) scale were estimated for 140 PD patients. And fasting plasma glucose (FPG) , 2 h postprandial plasma glucose (2hPPG) and glycosylated hemoglobin (HbA1 c) levels were measured in the PD patients, and compared with normal controls. Results There were 70 cases with normal cognitive ( normal cognitive group ) and 70 cases with abnormal cognitive ( abnormal cognitive group) in these PD patients. And there were 90 cases with H-Y 〈 3 (H-Y 〈 3 group) and 50 cases with H-Y≥3 (H-Y≥3 group) in these PD patients. The FPG and HbAle levels were significantly higher in normal and abnormal cognitive groups and in H-Y 〈 3 and H-Y ≥ 3 groups than those in normal control group (all P 〈 0. 05 ) , but the differences of 2hPPG level had no significance. There were no significant differences in the levels of FPG, 2hPPG and HbAlc between normal and abnormal cognitive groups as well as between H-Y 〈 3 and H-Y ≥ 3 groups. Conclusions No matter how cognitive dysfunction and disease severity is, the blood glucose levels in PD patients is elevated. There may be not obvious relationship between the disease severity, cognitive dysfunction in PD patients and blood glucose level.
