目的 分析64层螺旋CT (MSCT)图像,评价肺容积(LV)和肺密度指标与肺功能的(PFT)相关性.方法 选择46名吸烟者为研究对象,均于吸气和呼气时进行肺功能(PFT)检测试验,并接受胸部CT扫描.应用开源软件自动测量吸气和呼气的肺容积(LV)和肺密度(MLD),计算LV的呼气/吸气(E/I)比.结果 CT指标中,呼气MLD与肺通气功能密切相关(P<0.05),而且MLD E/I比值与肺通气功能指标也显著相关(P<0.05).吸气肺容积与肺通气功能不存在显著相关性;但呼气肺容积与肺通气功能显著相关(P<0.05);此外,肺容积的E/I比值与肺通气功能指标表现出显著的相关性(P<0.05).LV和MLD之间的相关性均较高(P<0.05),呼气肺容积与呼气肺密度的相关系数(R=-0.725),吸气肺容积与吸气肺密度的相关系数(R=-0.463).结论 螺旋CT中MLD测量值,特别是在呼气相的MLD与LV的测量值存在相关性.LV的E/I比值与MLD的E/I比值高度关联,两者均可以预测肺通气功能,适用于慢性阻塞性肺病的检查.
Objective To analyze 64 slice spiral CT (MSCT) image and evaluate the corrrlation of lung volume (LV), pulmonary density and pulmonary function (PFT). Methods 46 smokers were chosen as the research object. The lung function was tested during inspiration and expiration (PFT) test. All underwent chest CT scan. The inspiratory and expiratory lung volume, lung density (MLD) and the LV breath / suction (E / I) ratio were measured, Results The CT index and expiratory MLD were closely related with the pulmonary ventilation function(P〈0.05), and MLD E/I ratio and pulmonary function indexes were also significantly associated (P〈0.05). Inspiratory lung volume and lung ventilation function had no exist significant correlation, but the expiratory lung volume and lung ventilation function significantly related. In addition, the ratio of E/I and pulmonary function indices of lung volume showed significant correlation(P〈 0.05). The correlation between LV and MLD were higher(P〈0.05), the correlation coefficient of expiratory lung vol- ume and expiratory lung density (R=-0. 725), the correlation coefficient inspiratory lung volume and inspiratory lung density (R=-0. 463) ; Conclusion MLD measurements in spiral CT, especially in the expiratory phase of MLD, are as sociated with LV measurements value. LV E/I ratio and MLD E/I ratio are highly correlated, both of them can predict pulmonary ventilation function, which can be used in chronic obstructive puhnonary disease examination.
