目的:探索艾灸对实验性RA家兔关节滑膜液细胞因子水平的影响及其胆碱能神经调控机制。方法:将日本大耳白兔随机分为空白对照组、RA模型组、艾灸治疗组、艾灸治疗迷走神经切断组、艾灸治疗假手术组,用福氏完全佐剂塑造RA疾病模型,运用颈部左侧迷走神经干切断术作为干预手段,艾灸肾俞、足三里治疗,采用ELISA法检测滑膜液TNF-α、IL-1、IL-6、HMGB1含量。结果:与空白对照组相比较,RA模型组关节滑膜液中TNF-α、IL-1、IL-6、HMGB1含量显著增高(P〈0.01);与RA模型组比较,艾灸治疗组、艾灸治疗假手术组关节滑膜液中TNF-α、IL-1、IL-6、HMGB1含量明显降低(P〈0.01),而艾灸治疗迷走神经切断组无明显降低(P〉0.05);与艾灸治疗组比较,艾灸治疗迷走神经切断组滑膜液中TNF-α、IL-1、IL-6、HMGB1含量显著增高(P〈0.01),但艾灸治疗假手术组无明显增高(P〉0.05)。结论:艾灸能抑制实验性RA家兔关节滑膜液促炎细胞因子水平从而减轻RA滑膜炎症,其机制可能与兴奋胆碱能抗炎通路有关。
Objective:To explore the effect of cytokine level in RA rabbits'synovial fluid by moxibustion and regulatory mecha- nism of cholinergic nerves. Methods :Japan big-eared white rabbits were randomly divided into control group, model group, moxi- bustion treatment group, moxibustion treatment of vagotomy group, moxibustion treatment of sham operation group. RA animal mod- els were produced by Freunds'Complete Adjuvant(FCA) and left truncal vagotomy of the neck was as a means of intervention, moxibusting in "Shenshu ( BL23 ) " and "Zusanli ( 5T36 ) " acupoints. TNF - α, IL - 1, IL - 6, HMGB1 in synovial fluid were de- tected by enzyme -linked immunosorbent assay (ELISA). Results:Compared with the control group, RA model group's expressions of TNF-α, IL - 1, IL - 6, HMGB1 in synovial fluid were significantly increased( P 〈 0. 01 ). Compared with model group, moxibus- tion group and sham operation group were decreased significantly( P 〈0.01 )and vagotomy group was not decreased significantly (P 〉0. 05 ). Compared with moxibustion group, vagotomy group were significantly increased( P 〈 0. 01 ), but the sham operation group was not significantly increased( P 〉 0. 05 ). Conclusion:Moxibustion can inhibit pro-inflammatory cytokine level in RA rab- bits'synovial fluid and reduce the RA synovial membrane inflammation. Its mechanism may be related to activating cholinergic an- ti-inflammatory pathway.
