[目的]研究延边地区汉族青少年不同类型肥胖对代谢综合征(MS)及其组分患病危险的影响.[方法]选择2009年延边地区安图县高中3年级学生作为研究对象,通过问卷和体格检查的方式,调查研究对象的一般情况,测定身体质量指数(BMI)、腰围(WC)、腰围身高比值(WHtR)、血压(BP)、空腹血糖(FBG)、三酰甘油(TG)及高密度脂蛋白一胆固醇(HDL—C)等MS相关代谢指标,共收集1206份资料.BMI(kg/m^2)≥24为整体型肥胖,WHtR≥0.46为腹型肥胖.[结果]整体型肥胖和体质量正常组青少年的腹型肥胖检出率分别为43.0%和2.1%,高血压检出率分别为51.0%和24.2%,高血糖检出率分别为28.0%和11.3%,MS检出率分别为24.0%和1.0%,相比较差异均有统计学意义(P〈0.001).WHtR超标和正常组青少年的的整体型肥胖检出率分别为68.3%和5.7%,高血压检出率分别为55.6%和24.9%,高血糖检出率分别为44.4%和10.9%,MS检出率分别为38.1%和1.0%,相比较差异均有统计学意义(P〈0.001).多因素Logistic分析结果显示,整体型肥胖合并腹型肥胖、单纯腹型肥胖和单纯整体型肥胖(与无任何类型肥胖相比)青少年的不同代谢异常的患病危险间存在差异(P〈0.001),其OR值高血压分别为4.981,1.992,1.992,高血糖分别为8.307,2.077,0.545,MS分别为101.304,45.457,7.665.[结论]不同类型肥胖对MS及其组分患病危险的影响程度有所不同,而整体型肥胖合并腹型肥胖显著增大高血压、高血糖及MS的患病危险性.
OBJECTIVE To analyze the effects of different types of obesity to the risk of metabolic syndrome (MS) and its components in Han nationality adolescents, Yanbian area. METHODS The senior high school three grade students in Antu county, Yanbian area were selected in 2009 as research object, and it was measured by using the questionnaire and physical examination in 1 206 adolescents that body mess index(BMI), waist circumference (WC), waist to height ratio (WHtR), blood pressure and fasting blood glucose (FBG), triglycerides (TG), high density Iipoprotein cholesterol (HDL-C). BMI was ≥24 as overall obesity and WHtR was ≥0.46 as central obesity. RESULTS In overall obesity and normal body mass, the prevalence of central obesity was 43.0% and 2.1%, hypertension was 51.0% and 24.2%, hyperglycemia was 28. 0% and 11.3%, and MS was 1.0%, respectively, and there had differences between two WHtR overproof (≥0.46 24. 0% and significant groups ( P 〈 0. 001 ). ) and normal groups, prevalence of overall obesity was 68.3% In the and 5.7%, hypertension was 55.6% and 24.9%, hyperglycemia was 44.4% and 10.9%, and MS was 38.1% and 1.0 %, respectively, and there had significant differences between two groups (P〈0. 001). Logistic regression analysis showed that in groups with overall obesity combined with central obesity, alone central obesity and alone overall obesity, the OR value of hypertension was 4. 981, 1. 992 and 1. 992, hyperglycemia was 8. 307, 2. 077 and 0. 545, and MS was 101. 304, 45.457 and 7. 665, respectively, and the differences were statistically significant (P〈 0. 001). CONCLUSION The effects of different types of obesity to the risk of MS and its components are different, and the overall obesity combined with central obesity obviously increases the risk of hypertension, hyperglycemia and MS.
