目的:探讨宫颈上皮内瘤变(CIN)组织中P16蛋白表达及其与高危型人乳头瘤病毒(HR.HPV)病毒载量的相关性。方法:分别采用免疫组化sP法和第二代杂交捕获法(HC-2)检测宫颈(正常宫颈42例,CINI级32例,CIN1I53例,CINm92例)组织中P16蛋白表达及HR-HPV病毒载量。结果:①从正常宫颈、CINI、CIN1I到CINⅢ,宫颈组织P16蛋白表达强度及HR—HPV阳性率均递增,差异有统计学意义(P〈0.01)。②不同级别CIN组织中HR.HPV病毒载量分别呈正态分布,并随病变级别增高,病毒载量中位数增高,但病毒载量与病变级别之间无相关性(rs=0.1412,P〉0.05)。③P16蛋白表达强度与CIN级别呈正相关(rs=0.3633,P〈0.01)。④在CIN组织中,P16蛋白表达与HR—HPV病毒载量呈正相关(rs=0.4781,P〈0.01)。结论:P16、HR.HPV感染参与了CIN的发生与发展,P16免疫组化检测对宫颈癌前病变的早期诊断有很高的参考价值。
Objective:To examine the correlation between P16 and HR-HPV load in cervical intraepithelial neoplasia(CIN) and the potential clinic significance. Methods:Expression of P16 protein was detected by im- munohistochemical staining and HR-HPV load was detected by hybrid capture II (HC-2)in 177 specimens of CIN(32 cases of CIN I ,53 cases of CIN II and 92 cases of CIN III )and 40 specimens of normal cervix. Re- sults :The positive rates of P16 and HR-HPV in CIN were both significantly higher than those in normal cervix (P〈O. 01 ). The HR-HPV load had a normal distribution in each level of the CIN, and the median value in each level increased with the level increasing. There was no relationship between the HR-HPV load with the CIN level( rs = 0. 1412, P 〉 0. 05). P16 expression in CIN increased with the CIN level increasing ( rs = 0. 3633, P 〈0. 01 ). The expression of P16 was positively correlated to the HR-HPV load in CIN( rs =0. 4781, P 〈0. 01 ). Conclusions:P16 and the infection of HR-HPV contribute to development of CIN, and P16 has some value in early diagnosis of CIN.
