小学生焦虑抑郁共存的现状调查

目的:调查在校小学生,初步了解儿童焦虑抑郁共存现象的现状。方法:使用焦虑性情绪障碍筛查表(SCARED)和儿童抑郁障碍自评量表(DSRSC)调查长沙市某小学2—6年级的565名学生。结果:SCARED筛查阳性的有140名;DSRSC筛查阳性的有97名;两个量表都高于划界分的43名(7.61%)。在焦虑性障碍儿童中,30.71%合并抑郁;在抑郁障碍儿童中,44.33%合并焦虑。焦虑抑郁共存组在Achenbach儿童行为量表(CBCL)的退缩分量表得分(平均秩次297.7)高于单纯焦虑组(220.8)、单纯抑郁组(202.2),躯体主诉(268.2)高于焦虑组(232.2)、抑郁组(206.2),焦肜抑郁(277.2)高于焦虑组(227.7)、抑郁组(219.2),x^2=16.72—22.71,P〈0.01。自我意识量表行为分量表共存组(9.1±3.3)和焦虑组(9.7±2.9)低于抑郁组(10.6±3.2),焦虑分量表共存组(5.8±2.7)低于焦虑组(8.0±2.5)、抑郁组(9.2±3.0),合群(5.07±2.11)低于焦虑组(6.28±1.68)、抑郁组(6.63±1.84),幸福与满足(5.1±1.8)低于焦虑组(6.1±1.7)、抑郁组(6.0±2.0),F=4.45—74.34,P〈0.01。结论:在儿童中焦虑、抑郁问题存在较普遍,焦虑、抑郁共存比例高,焦虑抑郁共存儿童的内化性问题比单纯焦虑/抑郁者更严重,自我意识水平更低,在学校心理健康教育工作中应予以积极干预。

Objective: To investigate the comorbidity of depression and anxiety in primary school students. Methods :565 pupils ranged from 2 to 6 grade in a primary school were tested by The Screen for Child Anxiety Related Emotional Disorders (SCARED) and Depression Serf-Rating Scale for Children ( DSRSC). Results: There were 140 pupils with a total score higher than cut-off point of SCARED; 97 pupils with a total score higher than cut-off point of DSRSC ; 43 pupils with total scores higher than cut-off point of both SCARED and DSRSC, and the positive rate of comorbidity of anxiety and depression was 7. 61%. 30. 71 percent of children with anxiety disorder was considered comorbiding depression ; 44.33 percent of children with depressive disorder was considered comorbiding anxiety. Children with comorbidities had higher scores in Withdrawn(mean rank 297. 7 ) than Anxiety group(220. 8) and Depressive group(202. 2 ) ; Somatic Complaints(268. 2 ) than Anxiety group (232.2) and Depressive group ( 206. 2 ) , Anxious/Depressed ( 277.2 ) than Anxiety group ( 227.7 ) and Depressive group( 219. 2 ) of CBCL(x^2 = 16. 72 - 22. 71, P 〈 0.01 ). The subscale of "behavior" in Comorbidity group ( 9. 1 + 3.3 ) and Anxiety group ( 9. 7 ± 2. 9 ) had less scores than Depressive group ( 10. 6 ± 3.2 ) ; "Anxious" scores in Comorbidity group ( 5.8 ± 2. 7 ) less than Anxiety group( 8.0 ± 2. 5 ), and Depressive group ( 9. 2 ± 3.0 ) ; "Popular" ( 5.07 ± 2. 11 ) less than Anxiety group ( 6. 28 ± 1.68 ) and Depressive group ( 6.63 ± 1.84 ) , "Happiness and satisfy" ( 5.1 ± 1.8 ) less than Anxiety group ( 6. 1 ± 1.7 ) and Depressive group ( 6.0 ± 2. 0 ) of Piers-Harris Children 's Self-concept Scale ( F = 4. 45 - 74. 34, P 〈 0. 01 ) .Condustons: Anxiety and depression were generally occurred in children.
