目的探讨糖尿病患者眼球动脉血流动力学指标的变化及其临床价值。方法采用彩色多普勒成像技术,对40例糖尿病患者(无视网膜病变组与视网膜病变组各20例)与30例正常人的OA、CRA和CRV、CR~CRV流速比值的血流动力学变化进行检测。结果无视网膜病变组和视网膜病变组与正常人组相比,OA的内径变小,收缩期血流速度和舒张期血流速度均降低(P〈0.05),阻力指数增高(P〈0.05);视网膜病变组与无视网膜病变组和正常人组相比,CRA流速降低(P〈0.05);CRV流速在糖尿病无视网膜病变组增高(P〈0.05),而在视网膜病变组呈降低趋势。CRA/CRV流速比值变小。结论高频彩超可动态检测眼球后血流状态,对临床上评价糖尿病视网膜病变的程度有重要的参考价值。
Objective To investigate hemodynamic alteration of retrobulbar vessels in patients with diabetic retinopathy, and to explore its clinical significance. Methods The hemodynamics changes of ophthalmic artery (OA), central reti- nal artery (CRA), central retinal vein (CRV) and CRA/CRV velocity ratio were acquired by color Doppler flow imaging (CDFI) from 40 diabetic patients, including 20 patients with no diabetic retinopathy (NDR) and 20 patients with diabetic retinopathy (DR), and 30 healthy individuals. Results As compared with the control group, the diameter of OA decreased, systolic blood flow velocity and diastolic blood flow velocity were lower (P〈 0.05), and the resistance index increased in NDR and DR groups (P〈 0.05). As compared with NDR group and control group, CRA velocity decreased in DR group (P〈 0.05). CRV velocity increased in NDR group (P〈 0.05), while CRV velocity showed a decreased tendency in DR group. The ratio of CRA/CRV velocity was lower in NDR and DR groups than that of control group. Conclusions Hemodynamics changes of retrobulbar vessels in patients with diabetic retinopathy can be detected by CDFI, and it is helpful to evaluate the clinical diabetic retinopathy.
