目的 探讨噪声性听力损失(NIHL)患者椎动脉血流动力学的改变。方法 选择NIHL患者104例,其中听力轻度损失组24例、听力中度损失组42例和听力重度损失组38例,以及非噪声暴露人员80例(对照组),应用彩色多普勒超声测量各组受检者椎动脉收缩期峰值流速(PSV)、舒张末期流速(EDV)和阻力指数(RI)。结果 听力轻度损失组、中度损失组、重度损失组的PSV、EDV值均明显低于对照组,RI值均明显高于对照组(P均〈0.05),随着听力损伤程度增高,椎动脉PSV、EDV逐渐减低,RI逐渐升高(P均〈0.05)。结论 NIHL患者椎动脉血流动力学发生改变,听力损失程度越高,椎动脉血流动力学改变越明显。椎动脉血流动力学变化可作为评估听力损失程度的指标之一。
Objective To investigate the hemodynamic variations of vertebral artery in patients with noise-induced hearing loss( NIHL).Methods A total of 104 patients with NIHL were selected,including 24 cases in the mild hearing loss group,42 cases in the moderate hearing loss group and 38 cases in the severe hearing loss group. And 80 individuals without noise exposure were enrolled in the control group. The vertebral artery peak systolic velocity( PSV),end-diastolic velocity( EDV) and resistance index( RI) were measured in each group using color Doppler ultrasound. Results PSV and EDV were significantly lower and RI was significantly higher in the mild hearing loss group,moderate hearing loss group or severe hearing loss group compared to the control group( P〈0. 05). With the increase of hearing impairment,the PSV and EDV of vertebral artery decreased and RI increased gradually( P〈0. 05). Conclusion Hemodynamic variations of vertebral artery are observed in patients with NIHL. The hemodynamics of the vertebral artery changes more obviously when the degree of hearing loss is higher. Hemodynamic variations of vertebral artery can be used as one of the indices for evaluating the degree of hearing loss.
