目的探究子宫内膜异位症(EMs)患者内膜中骨桥蛋白(OPN)及基质金属蛋白酶9(MMP-9)的表达情况及二者的相关性,以及不同剂量孕激素对其表达的影响。方法选取2006—2015年在宁夏医科大学总医院住院治疗的符合纳入标准的EMs患者105例,术前1 d行诊刮术获得在位内膜(EMs在位内膜组),由卵巢囊肿剥除术或患侧附件切除术切除组织获得异位内膜(EMs异位内膜组)。另选取同期行输卵管绝育手术的患者90例作为对照,绝育手术前1 d行诊刮术获取子宫内膜(正常子宫内膜组)。采用免疫组化法检测3组内膜中OPN及MMP-9表达情况,并分析EMs在位内膜组、EMs异位内膜组OPN与MMP-9的相关性。2013年1—3月,从105例患者中随机选取9例患者的在位内膜组织进行后续试验,采用反转录聚合酶链式反应(RT-PCR)检测不同剂量(0、1、10、100、1 000nmol/L)孕激素干预后在位内膜腺上皮细胞中OPN、MMP-9 mRNA表达水平。结果免疫组化法染色结果显示,正常子宫内膜组在位内膜DAB染色后腺上皮细胞呈淡黄色,OPN、MMP-9均主要表达于内膜腺上皮细胞中;EMs在位内膜组DAB染色后腺上皮细胞染色加深,呈棕黄色;EMs异位内膜组腺上皮细胞染色进一步加深,呈棕黑色,OPN和MMP-9主要表达于EMs腺上皮细胞中。EMs异位内膜组OPN、MMP-9表达情况强于EMs在位内膜组、正常子宫内膜组(u值分别为7.553、-6.153、-9.754、-10.240,P〈0.001);EMs在位内膜组OPN、MMP-9表达情况强于正常子宫内膜组(u值分别为7.164,8.816,P〈0.001)。Spearman相关分析结果显示,EMs在位内膜组、EMs异位内膜组中OPN与MMP-9表达水平均呈正相关(rs值分别为0.657、0.745,P〈0.05)。0、1、10、100、1 000 nmol/L孕激素分别干预EMs腺上皮细胞后,其OPN及MMP-9表达水平比较,差异无统计学意义(P〉0.05)。结论 OPN、MMP-9在EMs患者在位、异位内膜中表达均增强,但不同剂量
Objective To investigate the expression and correlation of osteopontin ( OPN ) and matrix metalloproteinase 9(MMP - 9)in the endometrium of patients with endometriosis(EMs),and the influence of different doses of progesterone on their expression. Methods 105 patients with EMs,who received treatment in General Hospital of Ningxia Medical University from 2006 to 2015 and met the inclusion criteria,were selected in the study. The eutopic endometrium was got 1 d before surgery by curettage(EMs eutopic endometrium group). The ectopic endometrium was got through oophorocystectomy or tissue adnexectomy(EMs ectopic endometrium group). In addition,90 patients who had underwent sterilization operation of fallopian tube and the endometrium was got through curettage 1 d before sterilization operation( normal endometrium group). Immunohistochemistry was used to detect the expression status of OPN and MMP - 9 in the endometrium among the three groups, and the correlation of OPN and MMP - 9 between EMs eutopic endometrium group and EMs ectopic endometrium group were also analyzed. The eutopic endometrium of 9 patients were randomly selected from the 105 patients to carry out follow - up experiment, inverse transcription polymerase chain reaction(RT - PCR)was applied to detect the expression levels of OPN and MMP - 9 in the endometrial epithelial cells after progesterone intervention at different doses(0,1,10,100,1 000 nmol/ L). Results Immunohistochemistry results showed that the color of endometrium epithelial cells in eutopic endometrium after DAB staining in normal endometrium group was light yellow,and OPN and MMP - 9 were mainly expressed in endometrium epithelial cells.
