背景目前子宫内膜异位症(EMs)的病因和发病机制尚不清楚,治疗措施有限,严重影响女性的身心健康。目的探讨核转录因子κB(NF-κB)p65和骨桥蛋白(OPN)在EMs组织中的表达及二者的相关性。方法收集2013年1—12月于宁夏医科大学总医院妇产科因EMs行手术的患者35例(病例组),于术中或术后确诊,其中增生期17例,分泌期18例;收集同期行双侧输卵管结扎手术并经病理科诊断排除EMs的正常子宫内膜者30例作为对照(正常子宫内膜组),其中增生期15例,分泌期15例。分别采用免疫组化SP法、RT-PCR法检测正常子宫内膜组、EMs在位内膜组和EMs异位内膜组NF-κB p65、OPN及其mRNA的表达。结果 NF-κB p65、OPN主要表达于子宫内膜腺上皮细胞的胞质中,极少数表达于间质细胞中。EMs在位、异位内膜组NF-κB p65和OPN表达高于正常子宫内膜组,EMs异位内膜组高于EMs在位内膜组(P〈0.05)。3组增生期及分泌期NF-κB p65的表达比较,差异无统计学意义(P〉0.05),EMs在位、异位内膜组分泌期OPN的表达均高于增生期(P〈0.05);EMs在位内膜组和EMs异位内膜组中NF-κB与OPN的表达均呈正相关(r在位=0.88,r异位=0.78,P〈0.05)。NF-κB p65、OPN mRNA在EMs在位、异位内膜组中的表达高于正常子宫内膜组,EMs异位内膜组高于EMs在位内膜组(P〈0.05)。结论 NF-κB p65、OPN及其mRNA在EMs患者在位、异位内膜组织的表达明显增强,并呈正相关,提示二者极有可能通过相互激活进而诱导EMs的发生、发展。
Background The etiology and pathogenesis of endometriosis( EMs) is still unclear with limited therapeutic measures,which seriously influence the physical and mental health of women. Objective To investigate the expression of NF-κB p65 and osteopontin(OPN)in EMs tissues and their correlations. Methods 35 patients with EMs,who had been implemented operation in Department of Gynaecology and Obstetrics in General Hospital of Ningxia Medical University from January to December 2013,were selected as the case group,of which 17 are in proliferative phase and 18 in secretory phase;30 cases with normal endometrium,who had taken contraceptive operation at the same period and were diagnosed without EMs,were enrolled as a comparison( normal endometrium group),15 cases are in proliferative phase and 15 in secretory phase. Immunohistochemistry SP method and RT-PCR were applied to detect the expression of NF-κB p65,OPN and their mRNA in normal endometrium group,eutopic endometrium and ectopic endometrium groups respectively. Results NF-κB p65 and OPN were mainly expressed in the cytoplasm of endometrial glandular epithelial cells and few were expressed in stromal cells. The expression of NF-κB p65 and OPN in EMs eutopic and ectopic endometrium groups of EMs eutopic and ectopic endometrium groups were higher than that in normal endometrium group,their expression in EMs ectopic endometrium groups was higher than that in EMs eutopic endometrium group(P ﹤ 0. 05). There was no significant difference in the expression of NF-κB p65 proteins in proliferative phase and secretory phase within the three groups(P ﹥ 0. 05),the expression of OPN in secretory phase of EMs eutopic and ectopic endometrium groups were higher than that in proliferative phase(P ﹤ 0. 05);the expression of NF-κB p65 and OPN showed a positive correlation in EMs eutopic and EMs ectopic endometrium groups(rb = 0. 88 and rc = 0. 78,P ﹤ 0. 05) . The relative expression of NF-κB p65,OPN mRNA in EMs eutopic and EMs ectopic endometr
