目的尝试在SPF级sD大鼠胃内定植H.pylori,以建立大鼠H.pylori感染胃炎模型。方法选用sD大鼠30只,随机分为5组,即对照组及4个模型组,模型组分别予NaHCO,溶液、56%酒精、消炎痛溶液、NaHC03+消炎痛溶液联合H.pylori菌液灌胃造模,灌胃隔天1次,连续5次,并于最后一次灌胃结束后第2、4、6周处死大鼠,取胃黏膜组织分别记录损伤指数、进行快速尿素酶试验、胃黏膜涂片及病理切片。结果每组大鼠数、快速尿素酶试验及革兰染色阳性数:NaHCO,溶液+H.pylori组分别为6、4、4,56%酒精+H.pylori组分别为6、6、1,消炎痛溶液+H.pylori组分别为5、4、4,NaHC03+消炎痛溶液+H.pylori组分别为4、4、4。损伤指数及病理切片:对照组胃黏膜形态正常,无损伤,各模型组胃黏膜肉眼观察均有不同程度的损伤,病理切片观察第2周有少量淋巴细胞浸润,第4周大量淋巴细胞及中性粒细胞浸润,腺体破坏,为活动性胃炎,第6周有大量淋巴细胞及中性粒细胞浸润,但损伤无进行性加重。结论4种造模方式均可使H.pylori在SPF级大鼠胃内定植,定植时间至少长达6周,并能导致胃黏膜炎性损伤,损伤在第4周时最为严重,但以NaHC03+消炎痛溶液+H.pylori造模最为成功,值得推广。
Objective To establish the rat model of gastritis with H. pylori infection. Methods 30 SD rats were randomly divided into 5 groups, including control group and four model groups, model groups were given NaHCO3 solu- tion, 56% alcohol solution, indomethacin, NaHCOf indomethaein solution combined with H. pylori bacteria liquid in each group once every other day, 5 times in a row, rats were sacrificed after 2, 4, 6 weeks, and gastric mucosa damage index, rapid urease test, gastric mueosal smears and histologic sections were recorded. Results The rat number, rapid urease test positive and Gram stain positive number were as follows: NaHCO3 solution + H. pylori group were 6,4,4, H. pylori + 56% alcohol group were 6,6,1 , H. pylori + indomethacin solution group were 5,4,4, NaHCO3 and indom- ethacin solution + H. pylori group were 4,4,4. Injury index and pathologic sections: gastric mueosal morphology in control group was normal, no damage was found in the gastric mucosa. In model group, naked eye observation had var- ying degrees of injury, pathological observation showed there were a few lymphocytes in second weeks, four massive in- filtration of lymphocytes and neutrophils, glandular destruction, for active gastritis in fourth week, massive infiltration of lymphocytes and neutrophils, but injury without sex aggravate in sixth week. Conclusion Four model methods can set up the H. pylori infection rat up to 6 weeks, and can lead to gastric mucosal inflammatory injury. Injury in the 4th week is most serious, but NaHCO3+ indomethaein solution + H. pylori model is the most successful.
