目的 探讨磁共振动态增强扫描(DCE-MRI)信号强度一时间曲线(signal intensity-timecurve,SI-T曲线)对前列腺良恶性病变的鉴别诊断价值。方法对30例经病理证实的前列腺癌(Pca)患者和32例良性前列腺增生(BPH)患者行MRI平扫,动态增强扫描及延迟扫描,观察病变的MRI表现特征并分别绘制前列腺癌病灶和增生结节的SI-T曲线,重点分析其信号强度变化及SI-T曲线的走形趋势。结果 在磁共振动态增强扫描早期Pca癌灶和BPH增生结节均呈明显强化,有助于病变的检出。SI-T曲线参数:峰值时间Pca组早于BPH组,强化程度和强化率Pca组均高于BPH组(P〈0.05)。结论 磁共振动态增强扫描SI-T曲线对BPH和Pca的鉴别诊断有较明确的价值。
Objective To investigate the value of dynamic contrast- enhanced MR imaging (DCE- MRI) signal intensitytime curve ( SI - T curve) in the differential diagnosis between benign prostatic hyperplasia and prostatic carcinoma. Methods 30 patients with prostatic carcinoma(Pca) and 32 patients with benign prostatic hyperplasia(BPH) proved pathologically were imaged with conventional MR imaging, dynamic contrast -enhanced scanning and delayed scan. The SI -T curve of prostatic carcinoma and hyperplastic nodule were drawn respectively. The DCE - MRI signal intensity and the characteristics of SI - T curve were analyzed emphatically. Results In the early phase of DCE - MRI, all the Pea focuses and BPH nodules enhanced significantly. The enhancement parameters of SI - T curve : peak time of Pea was earlier than that of BPH, the enhancement degree and enhancement rate of Pca were higher than BPH those of( P 〈 0.05 ). Conclusion Dynamic contrast -enhanced MR imaging SI -T curve plays an important role in the differential diagnosis between BPH and Pca.
