目的:探讨夜班护理工作对神经外科ICU年轻护士注意功能的影响。方法:选取在神经外科ICU工作超过1年的年轻护士36名.分为夜班后检测组和夜班后充分休息检测组,同时选取年龄、受教育程度相仿的18名健康女性作为对照组.分别进行有关注意功能的神经心理学测评,了解夜班工作对神经外科ICU年轻护士注意功能的影响。利用听觉oddball范式对被试者进行刺激,采集脑电信号后进行P300分析.比较三组被试对新异(呼吸机报警声)和靶刺激所形成的P3a、P3b的差异。结果:夜班工作后.神经外科ICU年轻护士出现明显的注意功能下降.事件相关电位P300中P3a的潜伏期缩短、波幅升高,而P3b的潜伏期延长、波幅下降。经充分休息后,上述注意功能下降得到明显恢复。具有统计学意义(P〈0.05)。结论:夜班工作会引起神经外科ICU年轻护士注意功能下降。与P300的变化具有一定相关性.而夜班工作后护士经过充分休息注意功能会恢复。
Objective: To explore the effect of long-term nightshift to attention function of young nurses in neurosurgery ICU. Methods: 36 young nurses, who were working in neurosurgery ICU over one year, were divided into two different groups of detection after nightshift and detection for resting for 24 hours after nightshirt, mean- while 18 healthy women, similar to experimental groups in age and education, were chosen rigorously for control group. Then, we assessed the effect of long-term nightshirt to attention function of young nurses in neurosurgery ICU, by applying some neuropsychology testing on attention function. During the research, we employed audition oddball test to stimulate the subjects, analyzed information of electroencephalon gathered from subjects by P300, and to compare the disparations between P3a (one index of event related potential P300) and P3b (the other index of event related potential P300) to special stimulations (e.g. the warning of breathing machine) and targeting stimulations during the subjects from different groups. Results: After long-term nightshift, the attention function was decreasing in young nurses of neurosurgery ICU by neuropsychology testing, meanwhile it had a contrast to P3b. However, the attenuation of attention function was improved obviously after sufficient resting for 24 hours. Conclusion: The long-term nightshift could cause attenuation of attention function of young nurses in neurosurgery ICU, which was correlated to the diversities of P300, however, the attenuation could be improved by sufficient resting after nightshift.
