目的探讨脑震荡综合征患者脑功能网络小世界属性的变化情况及其意义。方法纳入安徽医科大学附属省立医院2013年7月至2014年8月接诊的脑震荡后3个月的患者24例,同期招募21名年龄、性别及教育程度相匹配的志愿者为健康对照组;采集两组的静息态功能磁共振数据,采用图论分析法构建功能性脑网络,分析两组脑网络的小世界属性(λ〉1,λ≈1,且α=γ/λ〉1时该网络具有小世界属性)及其参数——集聚系数(Cp)和平均最短路径(Lp)的变化。结果在稀疏度0.05~0.40取值范围,脑震荡综合征组和健康对照组均符合小世界属性(γ值分别为1.871±0.253、2.178±0.178,入值分别为1.105±0.050、1.083±0.015,λ值分别为1.654±0.249、1.942±0.155),但脑震荡综合征患者小世界属性较健康人群发生了变化(两组γ值差异具有统计学意义,t=3.350,P=0.030)。脑震荡综合征组的Cp值和Lp值均大于健康对照组(Cp分别为0.310±0.022、0.290±0.026,Lp分别为4.720±3.489、3.743±1.671),但仅Cp值差异有统计学意义(P〈0.05)。结论脑震荡综合征患者具有小世界属性,但存在局部颅脑结构的功能紊乱,整体脑网络功能并未发生改变。
Objective To investigate the change and its significance of the property of brain network small world in patients with post-concussion syndrome. Methods A total of 24 patients at 3 months after concussion admitted to the Department of Neurosurgery, the Affiliated Anhui Provincial Hospital, Anhui Medical University from July 2013 to August to 2014 were enrolled. Twenty-one age-, gender- and education-matched volunteers recruited in the corresponding time period were used as a control group. The resting-state functional magnetic resonance imaging (MRI) data of both groups were collected. The functional brain network was constructed using the graph theory analysis. The property of brain network small world (when λ〉1,λ≈1,and α=γ/λ〉1, the network has the property of small world) and its parameters -- the changes of the clustering coefficient (Cp) and the average shortest path length ( Lp ) of the 2 groups were analyzed. Results In the range of sparsity 0. 05 to 0.4, both the brain concussion syndrome group and the healthy control group met the property of small world (γvalue, 1. 871 ± 0. 253 vs. 2. 178±0.178; k value, 1. 105 ±0.050 vs. 1.083 ±0.015; σ value, 1.654 ±0.249 vs. 1.942 ±0. 155 ) , but the property of small world changed in patients with brain concussion syndrome compared with the healthy population, and there were significant difference in γ value between the 2 groups ( t = 3. 350, P = 0. 030). Both the Cp value and Lp value of the brain concussion syndrome group were more than those of the healthy control group ( Cp, 0. 310 ±0. 022 vs. 0. 290 ±0. 026; Lp, 4. 721 ± 3. 489 vs. 3. 743 ±1. 671 ), however, there was only significant difference in Cp value ( P 〈 0.05 ). Conclusion The patients with brain concussion syndrome have the property of small world, but they had the dysfunction of local brain structure, and the whole brain network function does not change.
