目的探讨改良背驮式肝移植术后。肾功能的变化及前列腺素对。肾功能的保护作用。方法比较32例下腔静脉阻断时间的不同(A组,下腔静脉阻断时间≤90分钟,B组:下腔静脉阻断时间〉90分钟,〈120分钟)对改良背驮式肝移植术后。肾功能变化的影响以及分析前列腺素E1对背驮式肝移植术后。肾功能的作用。结果①术后第1天,A组和B组患者的BUN、Cr较术前均有显著上升;术后第8天和第15天,两组患者的BUN和cr与术后第1天比较,均有显著下降(P〈0.05)。术后第1、8、15天,A组和B组患者的BUN、Cr相互比较,均无统计学差异(P〉0.05)。②前列腺素E1治疗组与常规治疗组相比较,术后第1天的BUN与cr的差异无显著性;术后第8天与术后第15天,前列腺素E1治疗组BUN与Cr的下降均更加显著(P〈0.05)。结论改良背驮式肝移植术后。肾功能可以逐渐缓慢恢复且下腔静脉阻断在120分钟以内,阻断时间的长短对术后的肾功能影响不大。背驮式肝移植术后早期使用前列腺素E1在一定程度上可以有效改善。肾功能。
Objective To discuss the characteristic changes and prognosis of renal function after modified piggyback liver transplantation and the protective contribution to renal function by using lipoprostagladin E1 agent after piggyback liver transplantation. Methods 32 patients who received modifying piggyback liver transplantation were divided into two groups according to the IVC clamping time: group A(n = 17, IVC clamping time ≤90m) and group B(n = 15, 90m 〈 IVC clamping time 〈 120m). Changes of renal function of the recipients after transplantation were analyzed. 36 patients who received modifying piggyback liver transplantation were divided into two groups: control group without prostagiandin E1 (n = 18 ) and treated group with prostaglandin E1 (n = 18 ). Changes of BUN and Cr of the recipients were analyzed on the first day before transplantation and the first day, the eighth day and the fifteenth day after transplantation. Results ① The value of BUN and Cr in group A and B increased significantly on the first day after modifying piggyback liver transplantation compared with that before transplantation; The value of BUN and Cr in group A and B decreased significantly on the eighth day and the fifteenth day compared with that on the first day after operation( P 〈0. 05 ) , while BUN decreased more significantly on the fifteenth day. The difference of BUN and Cr between group A and B has no statistic significance on the first day, the eighth day and the fifteenth day after transplantation( P 〉 0. 05). ②The value of BUN and Cr showed no significant difference between treated group with prostaglandin E1 and control group without prostaglandin E1 on the first day after operation, but the value of BUN and Cr in treated group with prostaglandin E1 deceased more significantly ( P 〈 0. 05 ) on the eighth day and the fifteenth day. Conclusions Renal function can recover gradually and the time of IVC clamping has little influence on renal function if the clamping time 〈 120 m af
