目的:研究冠心病患者血脂水平、糖代谢指标与冠脉病变程度的相关关系。方法:收集疑诊冠心病而行冠状动脉造影术的患者共计2 149例,其中确诊冠心病患者共2 009例,男1 515例(75.4%),女494例(24.6%)。冠脉造影下冠脉病变程度以Gensini积分表示,按积分的大小分成4个亚组,比较不同积分亚组之间的一般情况、糖脂代谢水平的差异,采用多元线性回归、有序Logistic回归方法,对冠心病患者冠心病的危险因素与冠脉病变程度进行相关关系研究。结果:在冠状动脉造影下确诊的冠心病患者的性别(男性)、吸烟、高血压、糖尿病、家族史等发生率显著高于对照组,年龄及BMI也显著高于对照组。糖、脂代谢指标中高TC、高TG、低HDL-C、高LDL-C、低apoA1/apoB比值、高Lp(a)、高Hcy、高FPG、高2hPG、高HbA1c的发生率也显著高于对照组,以上指标的平均值在不同积分分组之间也有显著差异。多元线性回归提示apoA1/apoB和HDL-C与Gensini积分呈负相关关系,HbA1c、年龄、Lp(a)、Hcy与积分呈正相关关系,尤以apoA1/apoB比值与Gensini积分关系最密切(β=-17,95%CI=-22.037~-11.964,P〈0.001)。有序Logistic回归分析提示,有冠心病家族史、高LDL-C、低apoA1、高Lp(a)、高2hPG为冠脉病变程度的危险因素,其中低apoA1是最为显著的危险因素(E=0.357,95%CI=0.167~0.547,P〈0.001)。结论:apoA1水平与冠心病患者冠脉病变程度关系密切,低apoA1可能是冠脉病变程度最显著的危险因素;其他影响冠脉病变程度的危险因素如冠心病家族史、高LDL-C、高2hPG、高Lp(a)亦需予以足够的重视。
Objective:To investigate relationship between blood lipids,blood glucose metabolism and coronary artery lesion degree in patients with coronary heart disease(CHD).Methods:We collected data from2 149 patients suspected with coronary heart disease been inspected by coronary angiography.2 009 patients were confirmed with coronary heart disease,among them there were 1 517 male patients(75.4%)and 494 female patients(24.6%).Coronary artery lesion degree was showed by Gensini scores.All participants were divided into four groups based on their Gensini scores.We compared the genera situation,blood glucose metabolism and blood lipids metabolism by multiple linear regression and ordinal logistic regression,and investigated the relationship between risk factors of CHD and coronary artery lesion degree.Results:The incidence rate in CHD group is higher than that in control group in terms of gender(male),BMI,smoking history,high blood pressure history,diabetes history,coronary heart disease history.Compared with control group,CHD group has higher TC,higher TG,higher LDL-C,lower HDL-C,lower apoA1/apoB,higher Lp(a),higher Hcy,higher FPG,higher 2hPG,higher HbA1 c,of which the means level is significantly different.Multiple linear regression shows the apoA1/apoB and HDL-C have negative correlation with Gensini scores,whereas HbA1 C,Age,Lp(a)and Hcy have positive correlation with Gensini scores.ApoA1/apoB is especially important(β=-17,95%CI =-22.037~-11.964,P〈0.001).Ordinal logistic regression shows that the CHD family history,higher LDL-C,lower apoA1,higher Lp(a),higher 2hPG are risk factors of coronary artery lesion degree,among which lower apoA1 is the most serious risk factor(E=0.357,95%CI =0.167~0.547,P〈0.001).Conclusion:The levels of apoA1 correlated to the degree of coronary artery lesion,and lower apoA1 is the most serious risk factor.Other risk factors such as CHD familly history,higher LDL-C,higher Lp(a),and higher 2hPG should be considered.
