目的探究足底压力改变与2型糖尿病患者周围神经病变病情的关系。方法以90例2型糖尿病患者作为研究对象,根据患者的病情分为无明显周围神经病变组(DM组22例),轻度周围神经病变组(DN1,22例),中度周围神经病变组(DN2,23例),重度周围神经病变组(DN3,23例),并选取同期体检无糖尿病者(DC组,60例),比较5组足底压力随时间的变化和患者足底各区域足底压力峰值。结果 1足底压力谷值:DM组谷值(47.57±2.46 min)明显高于DC组(49.11±3.06 min)(P〈0.05),DN1组谷值(28.13±2.43)明显高于DC组和DM组(P〈0.05),DN2组谷值(45.21±3.42 min)明显高于DC组和DN1组(P〈0.05),DN3组明显高于其余各组(P〈0.05);2第一峰值和第二峰值比:DC组、DM组小(P〈0.05),DN3组第一峰值、第二峰值(74.52±3.86 min)和谷值均比DC组、DM组、DN1组和DN2组大(P〈0.05);3与DC组比较,DM组、DN1组、DN2组和DN3组第一峰值和第二峰值出现时间出现较早(P〈0.05),DN2组和DN3组谷值出现时间提前(P〈0.05);与DM组比较,DN1组第一峰值时间较晚(P〈0.05),DN2组和DN3组谷值出现时间提前(P〈0.05);与DN1组比较,DN2组和DN3组谷值出现时间提前(P〈0.05);与DN2组比较,DN3组谷值出现时间提前(P〈0.05);4与DC组左足底各区域峰值压力比较:DM组T1、M3、M4和HM区域峰值压力较大(P〈0.05),M2区域分值较小(P〈0.05);DN1组T1、M3、M5、MF足底压力较大(P〈0.05),M2和HM区域峰值压力较小(P〈0.05);DN2组M3、M4、M5足底区域峰值较大(P〈0.05),T1、M2、HM区域峰值较小(P〈0.05);DN3组M2、M3、M4、MF足底区域峰值较大(P〈0.05),T1和HM区域峰值较小(P〈0.05)。5与DC组右足底各区域峰值压力比较显示:DM组T1、M3、M4、HM区域足底峰值压力较大(P〈0.05),M2、HM压力较小(P〈0.05);DN1组T1、M3、M4、M5、MF区域足底峰值压力较大(
Objective To explore the relationship between plantar pressure changes and peripheral neuropathy( PN) in patients with type2 diabetes( T2DM).Methods Ninety cases of T2 DM was divided into no PN group( DM,n = 22),mild PN group( DN1,n = 22),moderate PN group( DN2,n = 23),and severe PN group( DN3,n = 23).Sixty patients without diabetes( DC) were selected as control group.Changes of plantar pressure and peak plantar pressure were compared among the groups.Results 1 The valley values of plantar pressure in descending order were DC,DM,DN2,DN1 and DN3( all P0.05).2 The first peak and the second peak ratio was small in DC and DM( P0.05),they were larger in DN3 than other groups( P0.05).3 Compared with DC,earlier peak time was observed in DM,DN1,DN2 and DN3(P0.05),and valley appeared ahead of time in DN2 and DN3( P0.05); compared with DM,later first peak time was in DN1( P0.05)and earlier valley time in DN2 and DN3( P0.05); compared with DN1,valley time appeared earlier in DN2 and DN3( P0.05),and valley time in DN3 was earlier in comparison with DN2( P0.05).4 Compared with the peak pressure in left plantar regions in DC,peak pressure was greater in T1,M3,M4 and HM in DM,in T1,M3,M5 and MF in DN1,in M3,M4,M5 in DN2,and in M2,M3,M4 and MF in DN3( all P0.05); while the peak pressure was smaller in M2 in DM,in M2 and HM in DN1,in T1,M2 and HM in DN2,and in T1 and HM in DN3( all P0.05).5 Compared with the peak pressure in right plantar regions in DC,peak pressure was greater in T1,M3,M4 and HM in DM,in T1,M3,M4,M5 and MF in DN1,in M3,M4,M5 in DN2,and in M2,M3,M4,M5 and MF in DN3( all P0.05); while the peak pressure was smaller in M2 and HM in DM,in M2 and HM in DN1,in T1,M2 and HM in DN2,and in T1 and HM in DN3( all P0.05).Conclusion With the progression of PN in T2 DM patients,plantar pressure is transferred forward in foot and increased.
