超声斑点追踪技术定量评价维持性血透患者左心室旋转的研究

目的探讨超声斑点追踪技术在定量评价维持性血透患者左心室旋转的应用。方法以超声斑点追踪技术定量分析200例尿毒症患者(观察组)和200例健康体检者(对照组)的各项超声参数,比较两组受检者各室壁各节段的旋转角度值。结果(1)观察组IVSd、LVPWd、LVEDV和LVESV参数均显著高于对照组(P〈0.05),LVEF显著低于对照组(P〈0.05)。(2)两组的心底水平旋转角度基本保持一致(P〉0.05),观察组心尖水平旋转角度显著低于对照组(P〈0.05);两组的心底水平后壁、下壁及后间隔旋转角度均显著大于本组前间隔、前壁和侧壁旋转角度值(P〈0.05),观察组的心尖水平后壁旋转角度与前间隔、前壁具有统计学差异(P〈0.05)。结论超声斑点追踪技术可以清晰显示维持性血透患者左心室旋转较之健康体检者发生改变的情况。

英文摘要:

Objective To investigate the application of ultrasound speckle tracking technique in quantitative evaluation of left ventricular rotation in patients with maintenance hemodialysis. Methods The ultrasound parameters of 200 cases of uremic patients(observation group) and 200 healthy subjects(control group) were quantitatively analyzed by speckle tracking. The ventricular segmental rotation angle were compared between two groups. Results The parameters of IVSd,LVPWd,LVEDV and LVESV in the observation group were significantly higher than those in the control group(P〈0.05),while the LVEF was significantly lower than the latter(P〈0.05).The heart horizontal rotation angle of two groups was consistent(P〈0.05). The apical rotation of observation group decreased significantly compared with control group(P〈0.05); Posterior wall,inferior wall and posterior septum of the bottom of heart in the two groups were more than the rotation angle of the front wall and the side wall of two groups(P〈0.05),and the apical level of the anterior wall had significantly differences with the anterior septum,anterior wall in observation group(P〈0.05). Conclusions Ultrasound speckle tracking technique can clearly display the left ventricular rotation changes in maintenance hemodialysis patients compared with healthy people.
