目的:探讨磁共振弥散成像(DWI)和波谱分析(MRS)在前列腺癌诊断中的价值及两者的互补作用。方法:经穿刺活检、手术病理或随访证实的60例前列腺癌及前列腺增生患者,行MR常规扫描、DWI和MRS扫描,分男IJ测量前列腺癌区及正常周围带、移行带的表观弥散系数(ADC)值、(胆碱+肌酸)/枸橼酸[(Cho+Cr)/Cit]值,并分析比较不同组织及病灶间差异。结果:前列腺癌灶的ADC值明显低于正常周围带及移行带(P〈0.05),其95%可信区间为(0.720.80)×10^-3mm^2/s;前列腺癌CC/Ci值高于正常周围带、移行带(P〈0.05),其95%可信区间为0.97-2.13。结论:DWI和MRS两种方法在前列腺癌诊断中具有重要价值,两种方法各有优势,互为补充,联合应用可提高前列腺癌诊断的准确率。
Purpose: To evaluate the diagnostic value of diffusion-weighted imaging (DWI) and MR spectroscopy (MRS) in prostate cancer, and to investigate the complementation of DWI and MRS. Methods: Sixty patients, confirmed by biopsy, operation and follow-up, were included in study. All patients received DWI and MRS exams. The apparent diffusion coefficient (ADC) value and (Cho+Cr)/Cit ratio of prostate cancer lesion, peripheral zone (PZ) and transitional zone (TZ) were measured, respectively. Results were statistically analyzed with ANOVA. Results: The ADC values of prostate cancer foci were significantly lower than that of PZ and TZ. The 95% credibility interval was (0.720.80)×10^-3mm^2/s. The (Cho+Cr)/Cit ratio of prostate cancer foci was significantly higher than that of PZ and TZ. Its 95% credibility interval was 0.97-2.13. Conclusion: DWI and MRS are of great value in diagnosis of prostate cancer. Both of them have their own advantages and are complementary to each other. The combination application of DWI and MRS can improve the diagnostic accuracy of prostate cancer.
