目的探讨精神分裂症患者的认知状况及其临床症状与P300的相关性。方法将130例精神分裂症患者根据临床症状评分分为完全缓解组和未完全缓解组,同期选取65名健康志愿者设为对照组,采用阳性和阴性症状量表、韦氏智力测验和韦氏记忆量表对入组患者进行测评,同时对3组进行事件相关电位P300的测定。结果完全缓解组和对照组靶刺激P2、N2及P3的潜伏期显著短于未完全缓解组(P〈O.01),P2、P3波幅升高较未完全缓解组明显(P〈0.01);完全缓解组与对照组各项靶指标比较差异无显著性(P〉0.05)。完全缓解组韦氏智力测验、韦氏记忆量表各评分均显著高于未完全缓解组(P〈0.05或0.01)。完全缓解组阳性和阴性症状量表总分及各分量表评分与P300各指标均无显著相关(P〉0.05);未完全缓解组阳性症状量表评分与P300潜伏期和P300波幅呈显著负相关(P〈0.05),阴性症状量表评分与P300潜伏期呈显著正相关(P〈0.05)。结论精神分裂症患者存在认知功能损害,P300潜伏期和波幅可以作为其认知功能评估的客观指标。
Objective To explore the correlation among clinical symptoms, cognitive function and P300 in schizophrenics. Methods A total of 130 schizophrenics were divided into complete and non-complete re mission group according to the scores of clinical symptoms, simultaneously selectively 65 healthy volun- teers assigned to control group, assessments conducted with the Positive and Negative Syndrome Scale (PANSS), Wechsler Intelligence Scale (WIS) and Wechsler Memory Scale, (WMS), and P:~00 event-relat- ed potentials determined for three groups. Results P〈, N2 and P3 latency of were significantly shorter (P〈0.01) and the elevation of P2 and P3 amplitude more notable (P〈0.01) in both complete remission and control than non-complete remission group; there were no significant differences in each target index between complete remission and control group (P〈0.05). The WIS and WMS score were significantly higher in complete than non-complete remission group (P〈0.05 or 0.01). Correlation between total and each factor score of the PANSS and each index of P300 wasn't significant in complete remission group (P〈 0.05) ; the PANSS score was negatively related to P3 latency and amplitude notably and the SANS posi- tively to P〈 latency (P〈0.05) in non-complete remission group. Conclusion Schizophrenics have cogni- tive function impairment, P300 latency and amplitude could be used as objective index for assessing cognitive function.
