目的:通过三重刺激技术(TST)评估不同黄芪剂量的补阳还五汤对气虚血瘀型急性脑梗死(ACI)患者运动功能的影响,为中风病的中医药辨证治疗提供客观依据,探讨传统名方补阳还五汤在临床中的实际应用。方法:收集中医辨证气虚血瘀型急性脑梗死患者124例,运用TST技术检测患者患侧肢体TST剩余值从而了解早期皮质脊髓束损伤程度,通过中药辨证施治疗后分析不同黄芪剂量补阳还五汤患者TST剩余值检测结果的规律及变化程度。结果:(1)治疗前对照组与各组的TST值比较无统计学意义。(2)各组治疗前后TST差值比较组间有显著性差异,两两比较结果显示,对照组与各个治疗组间差异显著(均P〈0.01);30 g组与120 g组比较差异不显著,与60 g组比较有显著性差异(P〈0.001);120 g组与60 g组比较有显著性差异(P=0.004)。结合均数60 g组效果最好,其次为120 g组、30 g组。结论:TST能够客观、真实反映脑梗死患者中药治疗前后皮质脊髓束功能,为临床医生评估患者治疗后恢复程度提供可靠的数据评估;不同黄芪剂量补阳还五汤对气虚血瘀型急性脑梗死患者运动功能影响有差别。
Objective: To evaluate the motor function effects of Buyang Huanwu Decoction with different doses of Radix Astragali in acute cerebral infarction( ACI) patients with Qi- deficiency and blood- stasis via triple stimulation technique( TST). To provide objective evidences of the traditional Chinese medicine treatment and syndrome differentiation for patients who have stroke. To explore the clinical application of Buyang Huanwu Decoction which is a traditional famous prescription. Methods: A total of 124 cases of ACI patients with Qi- deficiency and blood- stasis by different diagnosis methods of traditional Chinese medicine were collected. The TST residual value in trouble side body of patients was detected via TST for the knowledge of early damage of corticospinal tract. The regularity and change of TST residual value in patients with different doses of Radix Astragali of Buyang Huanwu Decoction were analyzed after the TCM syndrome differentiation and treatment. Results:( 1) There was no significant difference between control group and other groups before treatment.( 2) Compared before and after treatment,the TST different value had significant difference among different groups. Compared with control group,the treatment grou Phad significant difference( P 〈 0. 01). There was significant difference between 30 g group and 120 g grou Pwhile there was no significant difference between30 g group and 60 g grou P( P 〈 0. 01). There was significant difference between 120 g group and 60 g grou P( P = 0. 004). The best effect was in 60 g group,next in 120 g and 30 g. Conclusion: TST can reflect the function of corticospinal tract objectively and accurately in ACI patients before and after treatment. It provides objective evidences for clinical doctors to evaluate the recovery condition after treatment. The influence on the motor function of ACI patients with Qi- deficiency and blood- stasis was different after treatment with different doses of Radix Astragali of Buyang Huanwu Decoction
