医养结合是老龄化国家针对老年公共服务中医疗服务和社会照护间＂双轨制＂而进行资源整合的一套方法和工具。自20世纪70年代以来,英国政府就致力于改变医疗服务与社会照护服务双轨制的状况,以适应人口老龄化的社会需求。基于此,通过梳理英国改革过程中的相关举措,总结其中的经验与教训,本文认为如果要实现服务的有效整合,必须在体系与制度设计中嵌入整合的理念,推广社区医疗与居家养老并探索整合照料的相关技术,统一预算筹资体系,推行分级诊疗制度,实现医养资源的有效衔接。

英文摘要：

Integrated care is a set of methods and tools,designed to combine health and social care in aging countries. Since the early 1970 s,the British government is committed to changing the track system between medical services and social care to accommodate the needs of society towards the aging population. Based on this,we list some related initiatives about the reform in the United Kingdom,and sum up the experience and lessons. To achieve effective integration of services,we must embed the concept of integration in the beginning of the system,promote the community health and home care,explore the technology related to integrated care,unify budget financing system and practise the grading treatment system.
