体外循环(CPB)是心内直视手术所必需的技术,然而术后会有严重的心肌缺血/再灌注损伤(MIRI)发生,此并发症直接影响术后心功能恢复。研究证明,CPB后的心肌存在胰岛素抵抗(IR)现象,其可导致心肌能量代谢出现异常紊乱,其中表现最为突出的是糖脂代谢紊乱。过氧化物酶体增殖物激活受体γ(PPAR-γ)是核内激素受体基因转录子,其主要参与糖脂代谢,还可增强心肌对胰岛素的敏感性,当PPAR-γ的表达水平升高时可减轻MIRI,降低心肌IR,进而对心肌起到一定的保护作用。
Cardiopulmonary bypass( CPB) is necessary for open heart surgery in clinical. However,myocardium will experience severe ischemia reperfusion injury( MIRI) which will seriously affect the recovery of heart function after surgery. Studies have shown that there is a phenomenon of insulin resistance( IR) after CPB,which may lead to myocardial energy metabolism disorder,and the most prominent result is glucose and fatty acid metabolism disorder. Peroxisome proliferator activated receptor γ( PPAR-γ) is a nuclear hormone receptor gene transcripton,mainly involved in the glucose and fatty acid metabolism,which also enhances the sensitivity of myocardium to insulin,so increasing the expression of PPAR-γ can reduce the MIRI and myocardial IR,and protect the myocardium to a certain extent.
