目的:评价电针治疗中风后吞咽障碍的临床疗效,分析目前研究现状并为指导临床提供参考。方法:计算机检索中国生物医学文献数据库(CBM)、中文期刊全文数据库(CNKI)、万方数据(WANFANG DATA)、维普数字期刊全文数据库(VIP)检索库以及国外数据库Pub Med、Embase、Cochrane自建库以来至2016年5月所有电针治疗中风后吞咽障碍相关的临床随机对照研究。由2位评价员根据纳入排除标准筛选文献、提取资料并利用Cochrane评分对纳入研究进行方法学质量评价。采用Review manager 5.2软件进行Meta分析。结果:共纳入文献12篇,累计患者824例。Meta分析结果显示,电针治疗组与对照组相比,总有效率[RR=1.34,95%CI(1.25,1.44),P〈0.000 01]、显效率[RR=1.68,95%CI(1.39,2.02),P〈0.000 01]、洼田饮水试验[WMD=1.07,95%CI(0.46,1.69),P〈0.05],差异均有统计学意义。结论:电针治疗中风后吞咽障碍具有很好的临床疗效,但受到纳入研究的数量及质量所限,该研究需要更多高质量的研究支撑。
Objective: To evaluate the clinical efficacy of electro-acupuncture in the treatment of dysphagia after stroke,to analyze the current research status,and to provide reference for clinical application. Methods:Randomized controlled trials on electro-acupuncture for dysphagia after stroke were retrieved from CBM,CNKI,Wanfang Data,VIP,Pub Med,Embase,and Cochrane from May 2016 since the establishment of the database. The literatures were screened by two reviewers according to the inclusion-exclusion criteria,and the methodological quality was assessed by Cochrane score. Review manager 5. 2 software was used for Meta-analysis. Results: A total of 12 literatures were included in the study,with a total of 824 patients. The results of Meta-analysis showed that there were significant differences between the electro-acupuncture group and the control group in terms of the total efficacy,effective rate,and Kubota water swallow test. Conclusion: Electro-acupuncture has a good therapeutic effect on dysphagia after stroke. However,the study needs more support with high-quality researches due to the limits of the number and quality of the included literatures.
