目的:探讨人血清生长分化因子-15(GDF-15)水平上升与非ST段抬高心肌梗死(NSTEMI)后心室重构程度的相关性。方法:选择NSTEMI患者120例为NSTEMI组,另选同期体检者120例为对照组,收集临床资料,应用酶联免疫法测定血清GDF-15。于入院后96h内及随访末(12个月)行超声心动图测定左心室舒张末期容积、左室收缩末期容积、左心室射血分数等指标。分析入院时2组血清GDF-15水平变化及与NSTEMI后心室重构指标的相关性。结果:NSTEMI组血清CDF-15表达水平显著高于对照组(P〈0.01);GDF-15与左室舒张末期容积(r=0.72,P〈0.01)、左室收缩末期容积(r=0.80,P〈0.01)、左室心肌质量(r=0.67,P〈0.01)呈正相关;与左室射血分数(r=-0.73,P〈0.01)、左室重构指数(r=-0.75,P〈0.01)呈负相关。多元线性回归分析,控制年龄、心血管危险因素及其他生化指标后,GDF-15水平仍与左室重构指数呈负相关(β=-0.821,P〈0.001)。结论:NSTEMI患者血清GDF-15表达水平升高,与NSTEMI后心室重构程度呈显著的、独立的相关性,提示GDF-15可能是预测NSTEMI后心室重构发生的可靠指标。
Objective:To study the correlation between serum levels of growth differentiation factor 15 (GDF- 15) and ventricular remodeling in patients with non-ST-segment elevation myocardial infarction (NSTEMI). Method: One hundred and twenty patients with NSTEMI as NSTEMI group and one hundred and twenty individu- als with physical examination as control group, collecte clinical data, serum levels of GDF-15 were measured by enzyme linked immunosorbent assay. The ventricular remodeling was assessed by echocardiographic within 96 hours after admission and follow-up at the end (12 months), measuring left ventricular end-diastolic volume, left ventricular end-systolic volume, left ventricular ejection fraction and other indicators of ventricular remodeling. A- nalysis the serum levels of GDF-15 expression in two groups at admission,and its relationship with ventricular re- modeling in patients with NSTEMI. Result: The serum levels of GDF-15 in NSTEMI group was higher than that in control group (P〈0.01) The serum levels of GDF-15 related to left ventricular end-diastolic volume (r=0.72, P〈0.01), left ventricular end-systolic volume (r=0.80,P〈0.01), left ventricular mass index (r=0.67, P〈0.01 ) and left ventricular ejection fraction(r= --0.73, P〈0.01), left ventricular remodeling index (r=- 0.75, P〈0.01). The negative association between GDF-15 and LVRI remained significant in amultivariate regression model adjusted for age,cardiovascular risk factors and other biochemical parameters (standardized β=-0. 821,P d0. 001). Conclusion: The serum levels of GDF-15 highly express in patients with NSTEMI. The serum levels of GDF-15 is independently associated with ventricular remodeling in the patients with NSTEMI, demonstrating that GDF-15 may be a reliable indicator for predicting ventricular remodeling in patients with NSTEMI.
